WEBER HUMAN SERVICES Case Manager - Youth Team (Early Psychosis Program) in OGDEN, Utah

Weber Human Services (WHS), located in Ogden, Utah serves a growing urban population residing in Weber and Morgan Counties. We are an organization dedicated to helping those in need rebuild their lives and reach their full potential. WHS was organized by a cooperative agreement between the Weber and Morgan county governments to serve as the local authority for the provision of their substance use, mental health and senior service programs. Weber Human Services is the only organization in the State of Utah providing these services under a single administration. Through dozens of human services programs, WHS helps thousands of its neighbors by supporting and empowering our most vulnerable groups including at-risk youth, the elderly, children, youth and adults suffering from mental illnesses, and those recovering from addictions. General job function and key job duties: • Provide targeted case management and skills development services to youth team clients and their families with mental health and behavioral issues. • Link, monitor, and coordinate client(s) and families to needed resources. Coordinate service delivery with interdisciplinary team as well as other community providers. • Advocate with and on the behalf of the client. • Work with clients in individual and group settings. Provide services in the office, schools and in the community. • Must complete and submit timely documentation of services according to Medicaid guidelines. • Attends mandatory trainings, team meetings, supervision and any other assigned duties.

This position includes group and individual skills development as well as engaging youth in activities in the community. Clientele will also be evaluated on a case-by-case basis to provide respite care as needed by. This position entails being part of a multi-disciplinary team working directly with youth, parents, legal guardians and other community agencies working with youth. This position is specific to the Early Psychosis Program.

Minimum Education/Experience/License/Special Requirements: Bachelor's degree in social work or related field preferred or 4 years of experience. • Certified Case Manager (or must obtain certification through Agency within 1 month of employment) • If SSW Licensed, must maintain license by meeting all requirements of licensure including obtaining requisite CEU's. • Must have a working understanding and experience working with youth with substance use disorders, co-occurring disorders, and knowledge of the criminal justice system. • Must have awareness of available community resources, effective communication skills, and collaboration skills. • Valid Utah Driver's License in good standing since this job will require driving for the agency.
