Amergis Hybrid Case Worker - Direct Hire in Novato, California
Amergis is hiring for a leading North Bay mental health services facility seeking a passionate Case Manager to empower clients towards independent community living.
Key Qualifications (HARD REQUIREMENTS):
Educational Requirement:
Associate's Degree with 6+ years mental health experience, OR
Bachelor's Degree with 4+ years mental health experience, OR
Master's Degree with 2+ years mental health experience
Responsibilities:
Develop personalized client service plans supporting recovery and independence
Provide comprehensive case management including:
Psychiatric rehabilitation
Symptom management coaching
Independent living skills development
Health and wellness education
Housing and budgeting assistance
Navigate complex healthcare systems
Facilitate community integration and peer support connections
Conduct medication management and adherence support
Provide crisis intervention services
Document clinical records and agency performance metrics
Essential Skills:
Strong communication across team and community providers
Cultural sensitivity
Crisis management capabilities
Ability to teach daily living skills
Proficient in MS Office
Valid California driver's license
Compensation
$1500 Sign-on Bonus Eligible
