Amergis Hybrid Case Worker - Direct Hire in Novato, California

Amergis is hiring for a leading North Bay mental health services facility seeking a passionate Case Manager to empower clients towards independent community living.

Key Qualifications (HARD REQUIREMENTS):

  • Educational Requirement:

  • Associate's Degree with 6+ years mental health experience, OR

  • Bachelor's Degree with 4+ years mental health experience, OR

  • Master's Degree with 2+ years mental health experience

    Responsibilities:

  • Develop personalized client service plans supporting recovery and independence

  • Provide comprehensive case management including:

  • Psychiatric rehabilitation

  • Symptom management coaching

  • Independent living skills development

  • Health and wellness education

  • Housing and budgeting assistance

  • Navigate complex healthcare systems

  • Facilitate community integration and peer support connections

  • Conduct medication management and adherence support

  • Provide crisis intervention services

  • Document clinical records and agency performance metrics

    Essential Skills:

  • Strong communication across team and community providers

  • Cultural sensitivity

  • Crisis management capabilities

  • Ability to teach daily living skills

  • Proficient in MS Office

  • Valid California driver's license

    Compensation

  • $1500 Sign-on Bonus Eligible

  • Comprehensive Benefits Package

    Location: Hybrid Role (Fridays WFH), Mon-Thurs On Site, San Rafael CA
