Overview

We are looking for a Case Manager PC to join our team. This position will directly report to the Branch Directorand is responsible for working collaboratively to assess, plan, facilitate and advocate for the options and services to meet a patient’s health needs.

  • Coordinates with the Case Manager Supervisor to complete the service plan authorization process

  • Coordinates all aspects of service delivery, ensuring that all services are delivered according to the service plan

  • Completes on-going monitoring with the client. The client is monitored for appropriateness of services, for health and safety issues, for re-evaluation of the service plan

  • Develops and completes the psychosocial assessment and Plan of Care and revises as needed.

  • Makes home visits per NC Medicaid guidelines to assure services are appropriate according to the Plan of Care

  • Collaborates with various disciplines to ensure a comprehensive approach to client care planning process.

  • Maintains strict confidentiality of client related information, staff, and physician matters at all times.

  • Provides counseling and emotional support to client to strengthen family/support system especially when disruptions arise.

  • Maintains cost effective service delivery by maintaining and utilizing knowledge of regulations and reimbursement requirements.

  • Promptly investigates complaints and/or errors and takes corrective action when warranted.

  • Documents Case Management in a running narrative in CAP beneficiary files.

  • Maintains manuals, policies and procedures, and other required administrative records on an on-going basis.

  • Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the clients served.

Compensation may vary within the salary range provided based on several factors including but not limited to a candidate’s location, experience, education, skills, licensure, certifications and department equity. Gentiva provides associates with a comprehensive benefits and total rewards package, of which base pay is just one piece.

Salary Range - $20/hr to $24/hr

About You

  • Bachelor’s degree in social work or a human service-related field from an accredited college or university.

  • 1 – 2 years of directly related community experience (preferably case management) in the health or medical field directly related to homecare, long-term care, or personal care.

  • The completion of a NC Medicaid certified training program within 90 calendar days of employment.

  • CPR Certifiction is required.

  • Knowledge of CAP/DA and CAP/C policies and procedures in preferred, but not required.

  • Must be self-directed, able to multi-task , able to alternate between office environment and home environments and deal with pressure and stress due to deadlines and client/family needs.

  • Potential for exposure to second-hand smoke, pet dander, variable temperature, and odors.

  • Occasional lifting and carrying supplies of 15-20 lbs. for short distances.

  • Must have reliable transportation and valid NC Driver’s license . Personal vehicle for traveling to client homes is a requirement.
