TEKsystems Case Management Associate in North Hollywood, California

SUMMARY OF RESPONSIBILITIES

The Case manager provides intensive, home and community-based case management services to medium to high acuity households transitioning out of homelessness. Responsibilities include intensive case management, assessments, goal and service planning, linking to resources, and crisis intervention for clients with complex trauma facing medical, mental health, and substance use barriers. This role involves collaboration with local community-based organizations and other service providers to assess housing stability, provide stabilization services, and coordinate multiple services to meet each client’s specific needs.

SPECIFIC DUTIES

  • Engage and develop rapport with households using Trauma-Informed Care, Harm Reduction, and Housing First principles.

  • Provide intensive case management services, including independent living skills, housing stabilization, money management, community integration, employment linkage, benefits establishment, and linkage to community providers.

  • Ensure clients have a current and comprehensive biopsychosocial assessment to create individualized goal plans, reviewed and updated quarterly.

  • Maintain accurate and current client files with all required documents and data.

  • Provide crisis prevention, crisis intervention, safety planning, and de-escalation supports.

  • Advocate on behalf of clients to address issues and barriers between landlords and participants.

  • Assist with referrals and linkage to eviction prevention and fair housing resources.

  • Attend mandatory trainings, seek out learning opportunities, and stay up-to-date on best practices.

  • Participate in the agency’s internship program and provide mentoring/coaching to new team members.

  • Participate in case conferences, staff meetings, and community meetings.

  • Perform additional tasks, projects, and responsibilities as assigned.

SPECIFIC DUTIES

  • Provide field-based stabilization services, requiring travel around LA County.

  • Assist participants with high acuity needs by connecting them to additional resources.

  • Work collaboratively with Property Management, Resident Advocates, onsite Co-Located Staff, and community agencies.

  • Organize and facilitate building-wide events for tenants.

ACCOUNTABILITY AND REVIEW

  • Reports to: Case Manager

  • Performance review: Annual

MINIMUM QUALIFICATIONS

  • Knowledgeable of issues faced by program participants, including health, substance abuse, mental health, domestic abuse, trauma, immigration, and legal matters.

  • Familiarity with best practices in homeless services is preferred.

  • Ability to engage with persons experiencing homelessness with empathy and non-judgmental attitude.

  • High tolerance and understanding for individuals presenting with mental health, substance use, and physical health needs.

  • Flexible and able to adapt in fast-paced environments.

  • Strong advocacy, conflict resolution, and time management skills.

  • Proficient in electronic data entry and Microsoft Programs.

  • Bilingual proficiency in Spanish/English is preferred but not required.

  • Ability to work a flexible schedule, including evenings and weekends.

  • Obtain and maintain CPR/First Aid Certification.

  • Ability to pass post-offer Tuberculosis (TB) clearances.

  • Regular and prompt attendance required.

  • Travel required 70% of the time.

  • Valid California Driver’s License and auto insurance in good standing.

  • Maintain confidentiality according to HIPAA standards.

  • Respond in a timely manner in all aspects of communication.

EDUCATION AND EXPERIENCE

  • At least one (1) year of work history and relevant transferrable skills.
