Atlantic Health System Home Care & Hospice Coord (RN) in NJ, United States

Responsible for working closely with hospital case management department to coordinate continued patient care with home care or hospice services after discharge. Assures quality customer service to patients, families, referral sources and co-workers. Serves as a consultant, educator, and customer service specialist to referral sources.

Principal Accountabilities:

  1. Develops initial plan of care along with physician, Home Care or Hospice Admission at home.

  2. Coordinates with patients, families, physicians, and discharge planning personnel, by obtaining complete clinical information for the hospice admission.

  3. Contacts Referral Manager or Assistant Manager to confer on complex clinical cases and appropriateness of referral acceptance.

  4. Ensures referral meets Home Care or Hospice criteria and that patient’s needs in the home are anticipated and appropriately addressed prior to discharge.

  5. Communicates with patients primary care physician in order to initiate Home care or Hospice services and obtain a verbal order for care.

  6. Maintains current knowledge of clinical issues, regulatory and reimbursement standards, insurance contract and rates.

  7. Performs other related duties as assigned.

Required:

  1. Bachelor's degree in Nursing, Social Work or related field.

  2. 3-5 years of experience as a Registered Nurse in Hospice, Palliative Care or relevant area.

  3. License for Registered Nurse.

Preferred:

  1. A completed apprenticeship or experience in a similar role.
