In this role, you will serve as a trusted advocate and liaison, seamlessly coordinating services with various agencies and educational institutions to provide a comprehensive support system for our youth. Your ability to effectively communicate and collaborate will be crucial in ensuring their educational, therapeutic, and aftercare needs are addressed holistically.

Key Responsibilities:

  • Coordinate resident placement needs and maintain effective communication with all involved parties

  • Complete required intake paperwork for youth within the specified timeline

  • Maintain accurate and timely records, and bill indirect client contacts weekly

  • Collaborate with the clinical team to coordinate school and vocational matters for residents

  • Schedule review meetings for all youth, complete review paperwork, and obtain necessary signatures

  • Transport and participate in youth court appointments and evaluations

  • Arrange and schedule youth visits and passes, communicating with relevant parties

  • Coordinate discharge planning and make appropriate referrals for support services

  • Attend meetings as requested, participate in team meetings, provide crisis management services, and obtain required staff training hours

Education and/or Experience Qualifications:

  • Bachelor's degree in Human Services or a related field is required

  • Experience working with children and adolescents is preferred

  • Possession of a valid driver's license and the ability to obtain a Chauffeurs license (Class E)

Required License/Certification (one of the following):

  • Chauffeurs license (Class E)

  • Excellent communication and interpersonal skills

  • Strong organizational and time management abilities

  • Proficiency in record-keeping and documentation

  • Ability to work collaboratively with various agencies and educational institutions

  • Compassionate and patient approach when working with youth

  • Commitment to maintaining confidentiality and adhering to ethical standards
