Job Overview:
Provides behavioral health consultations and short-term therapy utilizing a Team based model in the subspecialty adult clinics to adults and families in order to improve psychosocial functioning. Provides education to Residents and Medical Staff. Collaborates with Medical Staff, other members of Clinic Team and outside agencies. Provides advocacy for patients. Serve as liaison between patient and Medical Staff, promoting continuity of care. Provide referrals to community agencies. Conducts initial assessments, develops treatment plan and strategies provides individual, group, family, case management and psycho-education to patients/families as assigned. Completes all required documentation in a timely manner as well as staying current in all required competencies and licensure requirements. Schedules required amount of weekly appointments and maintains required amount of face- to face time with clients
Qualifications:
Required:
Masters level education, 2 years of experience in the field of human services
Preferably experience providing consultation and brief psychotherapy to adults in a medical setting.
Familiarity with the Cherokee/Integrated Behavioral Health Model and basic knowledge of chronic diseases preferred.

Essential Functions:
Demonstrates the ability to identify and facilitate groups that are clinically appropriate to the current population.
Develops formulation of the case incorporating presenting problem, and current life circumstances.
Develops treatment strategy and plan that is consistent with behaviorally specific long and short range goals
Is able to identify Performance Improvement Indicators and works toward achieving benchmarks.

Maintains License and Certifications on a timely basis and provides documentation.
Maintains regular collateral contact with agencies, schools and family members as appropriate
Provide weekly Productivity Report
Provides appropriate referrals to other facilities to ensure continuum of care.
Provides psychotherapy to individuals, families and groups in a professional manner.
Provides thorough intake assessments.
Submits all written reports in a timely manner.
- Actively participate in meetings that support the integrated model to provide comprehensive care to patients
Evaluate crisis situations and apply appropriate interventions
Formulates diagnosis and makes clinical treatment recommendations, including referrals to outside agencies
Provides consultations to adult patients during primary care medical visits using the Cherokee Model and develops strategies to assist patient in disease self-management, prevention, and treatment techniques to improve psychosocial functioning.
Screen patients for psychosocial, emotional/behavioral, and substance use through the use of screening tools to identify patient needs
