He resides with his elderly mother, who is an integral part of the care team. This role offers the chance to build a personal connection with your patient while using your nursing skills to improve their quality of life.Your Responsibilities:●Deliver professional nursing care following established care plans.●Administer medications and treatments as prescribed.●Monitor and assess the patient’s health, including vital signs and respiratory status.●Provide mobility support and assistance with daily living activities such as bathing and grooming.●Collaborate with the patient’s family and healthcare team to create and implement personalized care plans.●Maintain clear and accurate documentation of care provided.What We’re Looking For:●Licensed Professional: Current LPN licensure in New Jersey.●Experienced Caregiver: At least 1 year of nursing experience, preferably with individuals with developmental disabilities.●Ventilation Expertise: Experience caring for ventilator or respiratory patients.●Compassionate Communicator: Excellent communication and interpersonal skills.●Team Player: Ability to work collaboratively as part of a multidisciplinary team.●Patient-Centered: A genuine passion for improving the lives of others.What Makes This Role Unique:You’ll have the opportunity to provide highly personalized care for a patient in a home setting, working closely with the patient’s family to ensure their comfort and well-being.
