Title: Drug User Health Peer Navigator
Program: Health Care – Addiction Medicine and Drug User Health
Salary Range: up to $50,000
Position Overview:
Peer Navigator will join Project Renewal’s outpatient addiction medicine and drug user health team.
Project Renewal provides low-barrier health care services for people who use drugs and/or struggle with addiction. This care centers patient individual health goals and includes connecting patients to medication for opioid use disorder, safer use strategies, residential treatment, HIV/Hep C treatment and specialty medical care such as pain management. This care is offered in a patient-centered manner, supporting patient goals that range from managed or safer use to complete abstinence.
The Peer Navigator will work closely with our Nurse Care Manager of Drug User Health to outreach patients, identify patient goals and connect them with appropriate health and harm reduction services.
Essential Duties & Responsibilities:
The essential duties of thePeer Navigatorinclude but are not limited to the following activities:
Provides peer support and counseling
Collaborate with patient and medical team to identify care and treatment goals
Provide harm reduction education including safer use and overdose prevention to patients
Provide harm reduction and safer use supplies to patients
Refer patients to harm reduction and substance use services
Escort patients to harm reduction and substance use services as well as primary care, mental health care and specialty care
Support retention in care including community outreach, appointment reminders, check-ins etc.
Document services and support provided in electronic medical record.
Communicate with medical team including providers (MD, NP, PA) and Nurse Care Manager via phone, email and in case conferences.
Must be willing and able to navigate public transportation system to travel to shelter sites and community to outreach patients.
Qualifications:
High School Diploma or HSE
Minimum of 1 year working with people who use drugs and experiencing homelessness.
Maintains respect and compassion for all patients regardless of their active drug use or treatment compliance.
Knowledge and passion for harm reduction and supporting people who use drugs
Interest in medication for opioid use disorder treatment and education as well HIV/HCV prevention and treatment a plus.
Spanish language is a plus
All newly hired Project Renewal staff must provide verification of being fully vaccinated against Covid-19.
