The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to departmental budgets, qualifications, experience, education, licenses, specialty, and training. Position Summary

The division of Neuro-Oncology in the department of Neurology at the CUIMC is currently seeking a Clinical Coordinator II - RN Navigator. The Clinical Coordinator II - RN Navigator will work under the supervision of the NP Clinical Manager in caring for patients with various Neuro-Oncology diseases, and to coordinate care with multi-disciplinary specialists.

Responsibilities

  • Navigating the patient and family across settings and specialties according to the patient care plan
  • Bridging gaps between various specialists, services, points of care encountered by the patient/family
  • Coordinating expedited new patient visits and facilitating obtaining as well as reviewing of necessary records
  • Serving as a consistent primarily contact for patient/family
  • Serves as a constant point of contact through care continuum in collaboration with the admin team and per the direction of the providers as needed
  • Perform other duties per direction of providers (MD/NPs)
ACCESS: NEW PATIENT ACCESS
  • Working closely with the scheduling team to coordinate timely first visit that includes assisting with new patient record review and triaging to appropriate consult/specialty
  • Facilitate and assist with collection of patient medical records, pathology slides, and images in preparation for first visit and corresponding with provider with review of gathered records
  • Navigation for neurosurgery patients, particularly the inpatient/outpatient transition, and helping coordination for new outpatients
MEDICAL: CARE COORDINATION AND PATIENT REFERRAL
* Navigating care with multidisciplinary team including facilitating appointments with and access to providers outside of neurology or Columbia (including with radiation oncology, neurosurgery, heme-oncology, and with other specialties)
* Attends and participates in weekly multi-disciplinary tumor board meetings
* Anticipates and/or identifies potential logistical, financial, personal barriers to patient adherence to visit schedule and therapeutic plan;makes other care team members aware and interfaces with family members and any relevant internal and external providers to troubleshoot/develop a plan to minimize or eliminate obstacles
* Free drug program assistance with MERCK/BMS and coordinate with third party vendor
* Miscellaneous clinical tasks per direction of MD/NPs
EDUCATION PATIENT/FAMILY: Provides patient/family educational resources based on assessment findings relevant to teaching learning topics, styles and the identification of special needs and preferences
  • Provides patient/family with materials related to disease, treatment, potential adverse effects, symptom self-identification and management, parameters for distinguishing between what can be self-managed and what requires disease team intervention
  • Makes follow up contacts (e.g. by phone, email and visit) to ensure patient/family understanding of the plan of care, evaluated adherence levels and reinforced education provided
  • Documents patient/family response to information provided and communicates to the appropriate clinical team members
  • Coaches patient/family through diagnostic and therapeutic experiences to minimize anxiety and ensure adherence to medical plan; provides warm handoffs at various points but maintains contact with patient/family and point of care managers as patients move along the continuum

Minimum Qualifications

  • Bachelor's degree or equivalent in education and experience, plus four years of related experience as a Registered Nurse
  • Must have a current New York State Registered Nurse license and board certification
  • CPR/BLS certified

Preferred Qualifications

* Neurology or Neuro-Oncology experience in nursing
* Experience in supporting people with cancer and their families.
* Good communication skills and understands care coordination across disciplines.
* Excellent communication and organizational skills and a collaborative team approach to patient care.
* Bilingual (Spanish)
