Who We Are: CAMBA is a community of staff, volunteers, clients, donors, neighbors and partners who work together to build an inclusive New York City, where all children and adults have access to the resources and supports they need to thrive.  We take a comprehensive approach by offering more than 180 integrated programs in:  Education & Youth Development, Family Support, Job Training & Employment Support Services, Health, Housing, and Legal Services. Vincent Cyrus Plaza 57 Affordable supportive housing located in the East Harlem section of New York City. The units are studio apartments set aside for formally homeless and people living with HIV/AIDS.

Position: Case Manager

Reports To: Program Manager

Location: 57 East 128th Street, New York, N.Y 10035

What The Case Manager Does:

• Create and maintain tenant files/charts.

• Conduct initial assessments and quarterly reassessments.

• Work with clients to break through barriers to goals and to assist clients in advocating for themselves and moving toward self-sufficiency.

• Monitor clients progress toward their goals via regularly scheduled telephone contacts and/or face- to- face visits and document via progress notes

• Participate in case conferences

• Conduct groups/workshops twice monthly to assist clients with independent living skills

• Provide all required information for weekly/monthly/quarterly/annual reports

Minimum Education/Experience Required:

• Bachelor's degree and two years of applicable experience

Other Requirements:

• Experience working with persons with HIV, mental illness, and or substance abuse histories

Compensation: $45,000 annually

When salary ranges are listed, the range would represent the low and high end for the applicable position & program. The salary offered would be based on various factors unique to each program and candidate. This includes but is not limited to experience, education, budget and/or program size, internal equity, skills and other factors that may be required for the position and organization.

Status: Full-time (35 hours per week)
