Position Summary : The Guidance Center of Westchester’s Mobile Crisis Response Team (MCRT) was designed to serve individuals of all ages who are experiencing a behavioral health crisis. We are currently seeking an MCRT Practitioner to join our dynamic team. A MCRT Practitioner does not require a license, but must be able to provide professional staff (Licensed Clinicians) with back up duties within the Mobile Crisis Response Team. All crisis situations are designed as a team response between a clinician and a peer member. Through shared understanding, respect, and mutual empowerment, the peer worker helps the Individual in linking them with services. MCRT coverage must be available 24 hours a day, 7 days a week and 365 days a year. The team will be co-located with local police agencies and will be deployed through the local police department for immediate crisis response in the community. The mission of the MCRT is to deliver person-centered, trauma informed, culturally and linguistically competent services. These are high-quality, effective and integrated community-based services that promote resiliency, rehabilitation, and recovery.
Position Responsibilities:
Builds relationships with clients leading to the development of life goals and enhanced engagement in recovery.
Links clients to resources, services and natural supports internally at TGCW and in the external community to support attainment of recovery goals and reduction of isolation.
Works with clients to increase positive social connections and develop stronger family connections, improve life skills including obtaining necessary resources and empower the client to increase wellness self-management.
Documents all interactions in EMR.
Promotes linkage to services by assisting clients with the coordination of appointments, providing appointment reminders and escorting and/or driving to appointments based upon the client’s needs and preferences.
Will triage calls be able to assess an appropriate clinical response as authorized by the licensed clinician on the team.
Participate in program staff/case conference meetings to promote efficient communication and care.
Will assist in assessing individuals to provide them with the support needed to divert an ER visit or hospitalization .
The pr actitioner must be skilled in developing exceptional follow-up diversion planning for individuals served by the Mobile team.
A Mobile Crisis Response Practitioner does not require a license, but must be able to provide professional staff (Licensed Clinicians) with back-up duties within the Mobile Crisis Response Team.
Practitioners are required to be skilled in clinical triage and assessment of all emergency, urgent, and routine calls.
Qualifications:
-Bachelor’s degree in a Human Services field required
1-3 yrs. experience in a Human Service-related field and/or lived experience as a consumer of behavioral health services or experience with Mental Health population
Demonstrated peer support or mental health experience
