This is a field-based position that works throughout all of Pasco County.

• Location : Behavioral Health King Helie, Pasco County

• Status: Full-Time, Hourly

• Shift Hours: 8:00 am- 4:00 pm

• Shift: Days

• Weekend Work : No

• On Call: No The Case Manager is responsible for:

• Providing linkage, monitoring, planning and advocacy for the most appropriate mental health and community medical and social services.

• Coordinating the service delivery system for patients.

• Providing residents, faculty and community mental health practitioners with up-to-date available referral resources to provide continuity of care.

• Performing other duties as assigned. Required Education:

• Bachelor's in Related Field, Human Services Preferred Experience:

• 1 year in Social Work requiredOr - 1 year - Case Management required Required Certification/License:

• BLS (Basic Life Support)
