GENERAL SUMMARY
*
Under the general supervision of the Director of Clinical Services and/or the Senior Director of NLH Practices, apply the nursing process when making nursing judgments and decisions specific to assessment, planning, implementation/delivery, and evaluation of patient care, and documenting relevant data and care processes. Knowledgeable of and performs within the scope of practice as delineated by the NH Board of Nursing.

FUNCTIONAL DUTIES AND RESPONSIBILITIES

Clinical
Use effective interviewing and physical assessment skills telephonically or face-to-face to perform patient/family health status assessments by collecting and analyzing physical and psychosocial data, identifying needs, problems and patterns/trends, rendering an appropriate nursing diagnosis/intervention, and evaluating patient’s response to plan of care.
Exercise professional nursing judgment when performing a broad range of procedures and treatments, administering medications and prioritizing patient acuity as well as workload demands.
Provide patient care in a manner that demonstrates nursing knowledge, technical, critical thinking, and interpersonal skills competency, recognize changes in patient’s condition/situation and take appropriate action. Act as a role model in the performance of patient care.
Document in patient’s medical record all relevant data obtained in person or over the phone, including signs and symptoms, nursing assessment and diagnosis/intervention, instructions/advice given and patient/family’s response to instructions/care.
Evaluate responses to interventions and the effectiveness of the plan of care.
Provide health teaching and counseling to a defined group or population of patients and family members; advocate for patients, families, groups, and communities by attaining and maintaining what is in best interest of patient or group.
Assist clinicians during examinations, procedures, and treatments.
Assist and provide guidance to other staff members by acting as a nursing resource.
Appropriately delegate duties in accordance with NH Nurse Practice Act and professional standards of nursing practice to implement plan of care.
Promote team collaboration in meeting the needs of the patient/family.
Promote a work environment which fosters the acceptance of positive change initiatives.
Participate in identifying and implementing quality improvement activities that improve patient care outcomes, processes and promote healthy work team behaviors.
Acquire new knowledge and skills to remain current in practice.
Understand and follow universal precautions, infection control, and hazardous waste removal practices.
Comply with the Standards of Performance and general policies and procedures of the organization.
Willingly perform other duties as required or assigned.
