Company Overview and Mission Columbus House opened its doors in 1982 to provide services for men and women at least 18 years of age. Our goals quickly broadened from the mere provision?of food and shelter and simple survival, to understanding and working toward overcoming the problems which cause people to become homeless.

Purpose Under the purview of the Permanent Supportive Housing Department, the Recovery Support Coach provides support services to clients in Single Site Permanent Supportive Housing. The sites will include The Jefferson in New Britain and Meriden Commons in Meriden. The Recovery Support Coach performs a wide range of tasks to mutually support clients in gaining an understanding of recovery and wellness principles, practices, and pathways, including hope, choice, empowerment, recovery culture, and spirituality.

Duties and Responsibilities

  • Performs outreach duties within Meriden Commons (Meriden) and The Jefferson (New Britain) in an effort to identify and assist individuals and families suffering from a substance use disorder
  • Provide linkage and referrals to substance use services/treatment programs and other community resources
  • Assist clients in articulating and determining the steps they decide to take in order to reach their recovery goals
  • May provide transportation to appointments for clients at the discretion of the case manager on a case-by-case basis.
  • Assists clients in building relationships that will further enrich their daily life
  • Lead peer support groups for clients and families
  • Conduct educational workshops on opioid use disorders
  • Role model good self-care, coping skills, self-help strategies and the regular use of wellness tools
  • Ability to understand and accept different backgrounds while providing a non-judgmental approach to rendering services.
  • Attend weekly and monthly agency, program, and network meetings.
  • Monitor all cases assigned and complete all documentation/charting required by Columbus House
  • Expected to complete a variety of high-quality documentation (CARF Accredited, Department of Housing, Etc.)
  • Experience with HMIS (Homeless Management Information System) or ability to become proficient in database systems.
  • Create strength-based, person-centered service plans.
  • Complete all required assessments both electronic and in written form.
  • Ability to communicate efficiently and effectively with individuals served.
  • Must be able to set and maintain boundaries and adhere to confidentiality.
  • Coordinate and collaborate with other agencies and social service providers.
  • Other related duties as assigned

Qualifications

  • High School graduate or equivalent
  • Peer Recovery Support or Recovery Coach Certificate

``` - Ability to acknowledge and understand that they are only the experts on themselves and recognize the same for the people they serve

``` - Intimately familiar with mental health, substance abuse and dual diagnosis issues and group dynamics?

``` - Knowledge of and ability to effectively navigate substance abuse, mental health, and ocial services networks

``` - Knowledgeable of crisis intervention, trauma-informed care, boundaries, confidentiality, universal precautions, harm reduction and motivational interviewing

``` - Empathetic and compassionate, with individuals that have experienced homelessness and factors that lead to homelessness

``` - Excellent organizational and time management skills with a deadline-oriented mindset

``` - Strong written and oral communication skills employing the English language

``` - Proficient in Microsoft Office and other common desktop and online applications

``` - Familiar with criminal justice barriers and able to engage, motivate, and support the individuals serviced

``` - Able to work with multiple populations including youth (18-24 years old), adults, and families

``` - Basic knowledge of all aspects of the delivery of person-centered case management

``` - Able to meet all documentation requirements within set timeframes

```{=html}

``` - Ability to create professional and respectful relationships with clients and outside providers - Valid drivers' license with a good driving history

Reports To: Middlesex Supportive Housing Manager Schedule: Tuesday-Friday, 8:30am- 4:30pm Pay Range: $25.50-$27.50
