CASUAL/SEASONAL POSITION: Health and Human Service Case Manager I

Description: A Health/Human Service Case Manager I provides entry-level case management and eligibility determination to assist clients in meeting personal, social, health, or financial needs.

Duties Include:

Responsible for interviewing clients, family members, and other appropriate individuals to determine eligibility and health and human service needs. Work involves assessing personal, health, social, or financial needs in accordance with program requirements and coordinating with community resources to obtain client services. Work also includes preparing case histories, summaries, and logs of case activity. Knowledge of the principles, practices, methods, and techniques of health and human service work. Knowledge of eligibility and other applicable agency, state, or federal laws, rules, regulations, standards, policies, and procedures.

Location to be determined: New Castle County, Kent or Sussex.
