DESCRIPTION OF WORK An Aging Care Manager 2 recognizes the need for protective and supportive services for abused, exploited, or neglected older adults through a comprehensive assessment. Work includes obtaining information necessary for the identification of social, economic, emotional, health or physical needs and for assisting consumers in obtaining a variety of available services. You will complete Protective Services Basic Training and demonstrate an understanding of the protective services program, policies, and procedures. You will have the opportunity to work in close cooperation with other social agencies, hospitals, clinics, courts and community resources in planning to meet the needs of consumers. Developing person centered care plans to support older adults will be included in your duties.

Work Schedule and Additional Information: Full-time employment. Work hours are 8:00 am to 4:00 pm, Monday - Friday. On-call hours with this position. QUALIFICATIONS Minimum Experience and Training Requirements: Six months as an Aging Care Manager 1 or a County Caseworker 1;or Successful completion of the County Social Casework Intern program; or Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or An equivalent combination of experience and training.

Hiring preference for this vacancy may be given to candidates who live within Perry County. If no eligible candidates who live within Perry County apply for this position, candidates who reside in other counties may be considered. You must be able to perform essential job functions.

Legal Requirements: This position falls under the provisions of the Older Adult Protective Services Act. Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
