Position Details: Competitive Salary $4000.00 Sign on Bonus

Monday - Friday 8:00am - 4:30pm and included in the on-call rotation. NO WEEKENDS!

MUST HAVE LONG TERM CARE EXPERIENCE

Job Summary:

DAILY:

REPORT TO THE UNIT BY 0800 (UNLESS ANOTHER TIME HAS BEEN APPROVED BY THE DON) TO COMPLETE ROUNDS.

ROUNDS INCLUDE:

  • CHECKING ROOMS FOR CLEANLINESS/ SAFETY

  • BEDS MADE

  • NO GARBAGE ON FLOORS

  • NO LINENS LYING AROUND

  • NOTHING ON TOPS OF CLOSETS OR WINDOWS, ETC

  • ALARMS IN PLACE

  • WALKERS AND GAIT BELTS BEING USED

  • CHECK RESIDENT POSITIONING, STATUS, CONTINENCE, ETC

  • ADDRESS ANY ACUTE NEEDS.

  • MONITOR CLEANLINESS OF HALLWAYS, SOLARIUM, SHOWER ROOMS, MED ROOM, MED AND TREATMENT CARTS, UTILITY ROOMS, AND REFRIGERATOR

  • MEET WITH LPN ON UNIT FOR STATUS UPDATE.

  • CHECK STAFFING, ADDRESS ANY CONCERNS AND ASSURE ASSIGNMENT SHEETS FOR THE DAY TO MAKE SURE THEY ARE FILLED OUT PROPERLY WITH APPOINTMENTS, LUNCHES, AND OTHER DUTIES.

  • IF REQUIRED THIS INCLUDES TAKING MEDICATION CART AND DOING TREATMENTS.

  • CHECK DOCTOR ALERT BOOK/COMMUNICATION BOOK FOR ANYTHING PRESSING OR NEEDING TO BE ADDRESSED PRIOR TO MORNING REPORT (ALSO CHECK FOR HANGING NOTES) TO INCLUDE:

  • MD CALLS/UPDATES NEEDED

  • RN ASSESSMENTS NEEDED

  • F/U WITH RESIDENTS AND/OR FAMILY CONCERNS

  • ANY UNFINISHED ORDERS FROM PREVIOUS SHIFTS

  • PRINT 24 HOUR REPORT SHOULD INCLUDE THE FOLLOWING:

  • ANY INCIDENTS (3 DAYS)

  • CHANGE IN STATUS (UNTIL RESOLVED)

  • ABO (UNTIL COMPLETED)

  • NEW ADMISSION (7 DAYS)

  • TRANSFERS (3 DAYS)

  • CLUSTER ILLNESSES

  • MD APPOINTMENT RECOMMENDATIONS

  • CHECK THE LAB CALENDAR AND ENSURE ALL CRITICAL LABS AND PROTIMES ARE ADDRESSED FOR THE DAY, AND THAT NIGHT SHIFT OBTAINED THE ORDERED LABS.

  • MONITOR VS, TPR, I&O, CHECK WEIGHTS TO ENSURE THAT THEY ARE COMPLETED DAILY. ENSURE THAT THE LPN’S HAVE A BOWEL LIST.

  • MONITOR LPN’S/CNA’S ENSURE JOBS ARE COMPLETED AND ROUNDS ARE DONE IN A TIMELY MANNER AND THAT THEY ARE NOT TAKING EXCESSIVE BREAK PERIODS.

  • PHONE CALLS TO FAMILIES FOR UPDATES ON ANY CHANGES.

  • PHYSICIAN CALLS FOR NEW ORDERS ON RESIDENTS AS NEEDED.

WEEKLY DUTIES:

  • PERFORM QUALITY ASSURANCE AUDITS PER DON DIRECTION

  • ATTEND MEDICARE A MEETINGS

  • SWAT ROUNDS:

  • INITIATE SWAT SHEETS IF NEW AREA COMES UP AND NOTIFY PROPER DISCIPLINES

  • OBTAIN TREATMENT AND UPDATE CARE PLAN AS NEEDED

  • FOR WOUND ROUNDS ASSIST WITH WOUND CARE NURSE IN THE FOLLOWING

  • OBTAIN NEW TREATMENTS AS NEEDED

  • UPDATE SWAT SHEETS

  • ANY CHANGES SHOULD BE INCLUDED IN 24 HOUR REPORT

  • UPDATE CAN CARE PLAN AS NEEDED

  • OBTAIN CURRENT VITAL SIGNS, WEIGHT, AND REVIEW MEDICATIONS PRIOR TO MD VISIT FOR ACCURACY

  • AS YOUR QUARTERLY/ANNUAL/SIGNIFICANT CHANGE REVIEWS COME UP WITH THE MDS SCHEDULE, YOU MUST COMPLETE THE FOLLOWING:

  • CARE PLAN REVIEW FOR UPDATES AND CHANGES NEEDED (DONE PRIOR TO MEETING)

  • QUARTERLY NOTE WRITTEN IN PROGRESS NOTES (DONE AT MEETING)

  • CNA INSTRUCTION (KARDEX) REVIEW

  • FULL NURSING ASSESSMENT

  • ORDERS PENDING REVIEW

  • SCREENS

  • SKIN

  • FALL

  • PAIN

  • ELOPEMENT

  • AIMS (AS NEEDED)

  • RESTRAINTS (AS NEEDED)

  • REVIEW PALLIATIVE CARE GUIDELINES (AS NEEDED)

  • E-SCRIPTS FOR NARCOTICS MUST BE OBTAINED IF RESIDENT IS LOW OR OUT

  • THIS SHOULD BE CHECKED DAILY AND RENEWED IF RESIDENT HAS LESS THAN 48 HOURS WORTH OF MEDICATIONS.

  • MONITOR CHARTING AND PROVIDE GUIDELINES TO STAFF

  • MED A

  • ENSURE ALL ORDERS ARE CONFIRMED AND ADDED TO NEW ORDER CLIPBOARD

  • CREATE A PROGRESS NOTE IDENTIFYING IF THERE ARE ORDER CHANGES OR NO CHANGES AND ANY SPECIFIC REASONS FOR VISIT.

  • KEEP OPEN COMMUNICATION WITH STAFF AND ENSURE THAT THEY ARE KEPT UP TO DATE ON ALL CHANGES AND EXPECTATIONS.

  • STAFF DISCIPILINE AS NEEDED

  • COMPLETE STAFF EVALUATIONS

  • PRIOR AUTHORIZATIONS FOR MEDICATIONS

  • ATTEND MEETINGS AS ASSIGNED, IE: QA, PIP

  • MONITOR OMNICELL USE AND ENSURE THAT MEDICATIONS ARE ORDERED AS NEEDED

  • REPORT IS TO BE GIVEN TO NURSE ACCEPTING PATIENT

  • ALL BELONGINGS TRANSFERRED

  • ENSURE THAT THE CNA CARE PLANS/KARDEX, TAR AND MAR GO WITH RESIDENT

  • RESIDENT IS TO BE ORIENTED TO UNIT/ROOM/BATHROOM/ITEMS IN ROOM

  • WRITE NURSES NOTE

  • ADMISSIONS:

  • AS YOUR ADMISSION ARRIVES ON THE UNIT THEY NEED TO BE MET AND ORIENTED TO THE ROOM BY THE UNIT MANAGER AND OR THE FLOOR NURSE IF THE NURSE MANAGER IS UNAVAILABLE.

  • SKIN ASSESSMENT IS TO BE COMPLETED BY AN RN

  • USE ADMISSION ORDER SET TO ENSURE THAT ALL ORDERS ARE OBTAINED

  • THE FOLLOWING IS A LIST OF WHAT NEEDS TO BE COMPLETED:

  • ADD TO BATH LIST

  • DISCHARGES-

  • HAVE FACE TO FACE COMPLETED WITH PHYSICIAN IF NEEDED

  • D/C SUMMARY NEEDS TO BE COMPLETED BY PROVIDER AND OBTAIN ORDER FOR D/C

  • SCRIPTS TO BE OBTAINED AS NEEDED

  • DISCHARGE EDUCATION TO FAMILY AND RESIDENT INCLUDING MEDICATION TEACHING

  • MAKE F/U APPOINTMENT FOR AFTER D/C

  • REMOVE SECURE CARE IF NEEDED

  • WRITE PROGRESS NOTE TO SUMMARIZE STAY AND D/C

MONTHLY:

  • PHARMACY CONSULTS CALLED TO MD AND PUT INTO PLACE

  • MONTLY LABS SHOULD BE REVIEWED FOR CALENDAR PLACEMENT FOR THE FOLLOWING MONTH

  • EVERY RESIDENT SHOULD BE CHARTED ON A MINIMUM OF MONTHLY
