State of Massachusetts Adult Case Manager (Human Services Coordinator I) in New Bedford, Massachusetts

The Department of Mental Health (DMH) with the New Bedford Site Office is seeking a talented and experienced Adult Case Manager (Human Services Coordinator I) to serve as part of the DMH New Bedford Case Management team. The incumbent to this role will provide assessment, outreach, support, service coordination and person-centered advocacy to DMH eligible adults by promoting the agency’s philosophy of recovery, person-centered choices and cultural competence. The selected candidate will utilize strength-based goal setting and foster independent decision-making for persons served. Additional responsibilities include the development, implementation and monitoring of individual service plans and assisting individuals in securing services while performing all duties to reflect the highest level of respect and dignity for the persons served. _ _ _Please Note:_/ /This position is 40 hours per week. The selected candidate will work Monday through Friday from 8:30am - 5:00pm. The days off are Sunday and Saturday. _Duties and Responsibilities__ (these duties and responsibilities are not all inclusive):_ · Review eligibility determination record and obtain clinically relevant information through medical and psychiatric records, mental health assessments, school records as well as interviews with clients and service providers. · Develop Comprehensive Assessment and ISP for Persons Served, reviewing treatment progress regularly and making modifications to the ISP as necessary. · Schedule multidisciplinary team meeting to formulate recommendations for treatment and service and maintain consistent and effective collaboration and communication with service providers and colleagues. · Maintain regular contact with and communicate relevant information to clients, service providers, legal guardians and family members as appropriate and makes appropriate referrals when necessary. · Ensure that updated collateral information (for example, mental health assessments and legal documents) are included in client chart and notes are written on all significant contacts, including but not limited to, face to face visits, phone contacts with the client’s family/guardian (including contact relating to ISP content), incidents involving a client reported from a third party, all crisis events. · Maintain both electronic and paper medical records with up-to-date information, reflecting current bio-psych-social information on persons served. · Participate in meetings, case conferences __ _Required Qualifications:__ · Knowledge of psychosocial problems of those with mental/emotional disorders and the multiple handicapped. · Demonstrated skill in the principles, practices and techniques of counseling, interviewing, motivation and reinforcement. · Experience with the types and symptoms of mental/emotional disorders and the programs and services available to this population. · Expertise in the methods of general report writing. · Knowledge of principles and practices of crisis intervention and group process for counseling the mentally ill. · Ability to prioritize and multi-task duties based on client need, team and service needs. * * *Preferred Qualifications:*_ · For this function travel is required, therefore a current and valid Massachusetts Class D Motor Vehicle Operator’s license or the equivalent from another state is necessary. _About the New Bedford Site Office:_ The New Bedford Site Office provides case management services to adults and children.
