The Manager, Population Health, South under the direction of the Director of Care Coordination within Population Health Solutions, will be part of ACO Senior Management. In alignment with the strategic and operational goals of Hackensack Meridian Health, the position is responsible for providing leadership, planning, organizing, managing and monitoring the outcomes of the Care Coordination Team (Community Based Care Mgmt team members: RN, LPN, Health Coaches, Case Workers and Social Workers) for the ACO. Responsible for the development of guidelines and protocols which promote the development and implementation of a comprehensive program to achieve desired outcome measures. Identify improvement opportunities and facilitate process standardization to promote outcomes, maximize resources utilization and improve organizational efficiencies. Understands the clinical and financial aspects of ACO and is comfortable working with all levels of management and staff. As an essential element to the overall success of the ACO, the position will consistently demonstrate a strong work ethic and commitment to excellence.

This position will require travel to Southern Region practices; In office 4 days weekly every week. One day at home.

Responsibilities

A day in the life of a Manager, Population Health, South at Hackensack Meridian Health includes:

  • Designs, develops, implements, evaluates and revises Care Coordination guidelines for input and output management to ensure optimum outcomes for care coordination for patients.

  • Directly responsible for the care coordination processes in assigned off-site physician offices.

  • Conducts care coordination needs analysis and develops an implementation plan for assuring continuation of optimal care and outcomes during transitions in care.

  • Monitors Care Coordination team to assure compliance with established Care Coordination guidelines.

  • Offers creative suggestions to program development obstacles.

  • Collaborates with other members of the ACO/CIN management team to develop policies and procedures to support systems, standards of practice, and evidence-based practices.

  • Provides daily support and subject matter expertise to ACO/CIN and Medical Center Leadership and identifies areas of opportunity for continuous improvement including on-going education and resources as needed.

  • Develops care coordination tools needed to streamline patient care and reporting.

  • Critically evaluates existing practice standards based on current research findings and integrates changes into practice. 10. Other duties and/or projects as assigned.

  • Adheres to HMH Organizational competencies and standards of behavior.

Qualifications

Education, Knowledge, Skills and Abilities Required:

  • Bachelor's degree in Nursing.

  • Minimum of 5 years of experience with direct involvement in compliance activities.

  • Minimum of 3 to 5 years of leadership experience.

  • Excellent interpersonal skills, communication and creative problem solving.

  • Knowledge and understanding of governmental and JCAHO regulations.

  • Proficient use of software conductive to developing and presenting program deliverables, i.e. Microsoft Office Suite.

Education, Knowledge, Skills and Abilities Preferred:

  • Master's degree in Nursing.

Licenses and Certifications Required:

  • Current NJ Registered Nurse license in good standing.
