The Clinical Coordinator Registered Nurse will play a pivotal role in ensuring high-quality care for new and transferred patients at the Infectious Disease (ID) clinic. This position involves comprehensive nursing assessments, coordination with the medical team, and active participation in multidisciplinary meetings to ensure seamless patient care and follow-up.

Responsibilities

A day in the life of a Clinical Coordinator, Registered Nurse at Hackensack Meridian Health includes:

Nursing Assessments:

  • Conduct thorough nursing assessments to identify patient needs upon their arrival at the clinic.

  • Establish and update care plans for new and transferred patients in collaboration with the medical team.

Patient Screening and Vital Signs:

  • Assist the medical team in screening patients and obtaining vital signs.

  • Inform providers of patient status and any immediate needs requiring urgent attention.

Multidisciplinary Collaboration:

  • Play a key role in multidisciplinary meetings held after each clinic session.

  • Follow up on patient care plans and referrals, ensuring continuity and coordination of care.

  • Work closely with the Clinical Coordinator MSW to collaborate on patient care and resource allocation.

Patient and Provider Communication:

  • Facilitate clear and effective communication between patients and providers.

  • Educate patients and their families about their health conditions, treatment plans, and available resources.

Documentation and Reporting:

  • Maintain accurate and detailed patient records, ensuring all documentation is up-to-date.

  • Report on patient progress and outcomes to the medical team and in multidisciplinary meetings.

  • Documents patient care in Electronic Medical Record (EHR) for grant reporting purposes.

  • Strong assessment, communication, and organizational skills.

  • Ability to work effectively in a multidisciplinary team.

  • Follow-up care with patients enrolled in the program.

  • Other duties and/or projects as assigned.

  • Adheres to HMH Organizational competencies and standards of behavior.

Qualifications

Education, Knowledge, Skills and Abilities Required:

  • BSN

  • One year (1) previous experience in an ambulatory care setting or similar healthcare setting.

  • Excellent written and verbal communication skills.

  • Proficient computer skills that may include but are not limited to Microsoft Office and/or Google Suite platforms.

Education, Knowledge, Skills and Abilities Preferred:

  • CareWare experience and EPIC

  • Experience with Pediatrics and Perinatal care

  • Three (3) years of professional nursing experience

Licenses and Certifications Required:

  • NJ State Professional Registered Nurse License.
