Allegheny Valley Association of Churches, Inc. Self-Sufficiency Case Manager in NATRONA HEIGHTS, Pennsylvania

Time Requirements

§  40 Hours per week: Monday-Friday between the hours of 9:00 am to 5:00 pm

§  Occasional weekends and evenings may be needed with flex time necessary to accommodate participants' needs

§  As necessary to complete the job responsibilities

Job Requirements

§  A minimum of a Bachelor's Degree in social work, workforce development or related fields and/or it's equivalency in experience.

§  Valid PA driver's license with reliable vehicle

§  Have or be able to obtain and maintain mandatory clearances

§  Ability to case manage program participants adequately through both office and home visits

Purpose

§  To be responsible for the supervision, organization, and coordination of the activities of the AVAC Brighter Future Self-Sufficiency program.

§  To provide case management and self-sufficiency building services to low-income individuals and families seeking assistance with education, job training and employment.

§  This is a highly responsible professional position providing assessment, management, and advocacy services to program participants.

Skills Needed

§  Proficient in Microsoft applications: Word, Excel, Outlook

§  Ability to learn and maneuver the county database for CSBG funded programs.

§  Must be assertive with excellent verbal and written communication skills.

§  Ability to relate to program participants and establish a positive rapport in a respectful, non-judgmental manner.

§  Must be able to work independently, as well as effectively within a team.

§  Knowledge of Allegheny County community services resources.

§  Must have a working knowledge of workforce development resources and be able to refer appropriately.

§  Ability to handle stressful situations in a reasonable, calm manner and be able to always maintain professionalism and confidentiality.

§  Must be well organized and have the ability to keep and maintain files as needed.

§  Must be able to attend workshops, trainings, and meetings as needed.

§  Willingness to travel locally.

Responsibilities

  •  Perform general case work services for all adults enrolled in the program
  • To engage clients in the self-sufficiency process and the steps necessary to achieve this goal
  • Responsible for maintaining daily records of clients
  • Responsible for writing progress notes, discharge summaries, and creating individualized goal plans
  • Provide and manage referrals to other programs and advocate on behalf of the participant when necessary
  • Maintain a file for each client in Synergy in accordance with requirements from funders and the agency.  Files should consist of, but are not limited to: All required intake information, Individualized goal plan, Detailed progress notes, Financial literacy: budgets, credit scores, etc., and all written correspondence and documentation of any financial assistance given.
  •  To record and do research for all requests for financial assistance before submitting them to the Executive Director for approval.
  • Provide financial education services to enrolled participants including budgeting, obtaining credit reports, assisting with credit repair plan, financial goal setting and effective follow-up services.
  • Develop knowledge of and working relationships with potential employers and other employment resources (CareerLink, etc.)
  • Provide a positive role model to participants, offering support and guidance in life skill development (e.g., parenting, employment, budgeting, and housing concerns). Serve as an advocate with community service agencies.
  • Conduct exit interviews with participants leaving BFSS.
  • Attend community wide meetings sponsored by the Allegheny County Dept. of Human Services and other social service committees as related to position, on an as-needed basis.
  • Work cooperatively with other agency staff
  • Provide monthly reports to the Allegheny County Dept. of Human Services, and all other reports requested.
  • Provide a monthly report to the Executive Director for the Board of Directors meetings.
  • Provide a report to the Executive Director for the annual General Assembly and Planning Session.
  • Assistance at fundraising and special events as needed.
  • Other duties may be assigned at the direction of the executive director.
