Amergis Healthcare Staffing is seeking a Clinical Admissions Coordinator responsible for the coordinationand planning for pre-admission activities. The Clinical Admissions Coordinator identifies and collectsinformation, with the assistance of other departments and hospitals, to finalize the admissions process.
Essential Duties and Responsibilities:
Assigns and facilitates patient placement
Coordinates patient admissions functions
Notifies facilities of admission decisions in accordance with clinicalguidelines
Identifies medical record information that is needed from referringfacility
Collects and distributes admission information per facility/hospitalguidelines
Assists in communicating and explaining admission information topatient, families and referral sources
Facilitates admission of patient(s) referral sources
Responsible for managing facility/hospital capacity
Facilitates physician-to-physician communication as applicable
Informs facility/hospital staff of new patient admissions
Contributes in the development of policies, objectives and strategies
Participates on internal committees as applicable
Participates in staff meetings
Complies with patient confidentiality and HIPAA standards
Performs other duties as assigned/necessary
Minimum Requirements:
Licensed healthcare professional (current and valid license in state ofassignment)
One (1) year healthcare experience highly preferred
Clinical experience in hospital setting preferred
Ability to apply clinical guidelines to pre-admission records
Ability to perform clinical assessments to evaluate patient care needs
Ability to work collaboratively with medical staff
Experience in Case Management, Utilization Management or DischargePlanning preferred
Ability to effectively elicit/provide information to and fromappropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communicationskills; proficiency in the English language may be required
Computer proficiency required
Must be at least 18 years of age
