GENERAL RESPONSIBILITIES

Works closely with administration, medical providers, primary care teams, and social services agencies to provide short-term care coordination and connection to resources to support clients in improving their health and general well-being. The Patient Navigator (PN) Supervisor provides direct supervision and subject matter expertise to patient navigators to meet organizational priorities and to meet grant goals. Responsible for developing the Patient Navigator program and implementing in reach and outreach strategies to improve affordable access to Terry Reilly\'s services. The PN Supervisor will be responsible for providing reports, or as requested, showing the impact of the PN program. The PN Supervisor assists with team training to promote and strengthen the knowledge base for PNs.

MINIMUM QUALIFICATIONS

  1. Three years post-secondary education or three years\' experience in lieu of education.
  2. Excellent communication and customer service skills.
  3. Good understanding of health care delivery, terminology, and processes.
  4. Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community.
  5. Expertise on coverage options like the state\'s Health Insurance Exchange including but not limited to: Medicare, Medicaid, and the Children\'s Health Insurance Program (CHIP).
  6. Expert knowledge of community agencies and resources.
  7. Available for regular weekend and evening duties as needed.
  8. Demonstrated ability and experience to do project management.
  9. Must have reliable transportation to get to different outreach sites.
  10. Self-starter who functions independently with minimal supervision.
  11. Well-organized, creative, flexible, open-minded, and able to manage several tasks simultaneously.

PREFERRED QUALIFICATIONS

  1. Extensive knowledge of current law and rules regarding Medicare, Medicaid, and SSA. Ability to navigate CMS, IDAPA Rules, and POMS manuals to verify these rules.
  2. Strong Working Relationships with Programs Partners such as SSA, CMS, IDHW, and DOI,
  3. Bachelor\'s Degree in health promotion or other health related field, education, communication, or social services.
  4. Some clinical experience, e.g., CNA, MA, or phlebotomy.
  5. Bilingual in English and Spanish.
  6. Knowledge and experience with health promotion/disease prevention approach to chronic diseases.
  7. Community health experience.
  8. Experience working with underserved and/or aging patient populations.
  9. Two years\' experience with Medicare and major insurance carrier regulations, procedures, and benefit plans.
  10. Experience using Electronic Health Record Systems, including scheduling and reporting.
