Position Summary

The Legal Services Coordinator (LSC) will perform duties required to develop, implement and sustain coordinated medical-legal partnership activities at Zufall’s Wellness Center located in Morristown, NJ. The HEAL medical-legal partnership is a cooperative project of Zufall Health and Legal Services of Northwest Jersey (LSNWJ) that has been funded by Morris County’s Department of Human Services. The LSC will work closely with Zufall’s skilled multidisciplinary team composed of Medical Providers, Medical Assistants, Licensed Clinical Social Workers, Oral Health providers, Outreach team and support staff. The LSC will provide coordinated services, ensuring that Zufall patients are linked to legal assistance by LSNWJ attorneys. LSC will work along with the clinical team to identify and connect patients, The services provided will help enhance access to legal counsel as a primary social driver of health and will coordinate these legal services with LSNWJ’s staff as well as obtaining, entering, and compiling clinical and program data in concert with Zufall’s data analyst and quality assurance coordinator. The LSC will help link unaccompanied minors to legal services and coordination of care.

The Legal services coordinator will serve as the link between Zufall patients, staff and Zufall’s two Medical Legal Partnerships (MLP). These innovative collaborations seek to address previously unmet social needs and to enhance the enabling services offered to Zufall patients. Specifically, the HEAL MLP has served since 2018 to link Zufall patients, particularly those with mental health or addiction diagnoses, to essential legal services. Florecer, Zufall’s newest MLP, is a collaboration between Zufall Health and the New Jersey Consortium for Immigrant Children (NJCIC) and is directed at legal services for Unaccompanied Children recently arrived to New Jersey.

The LSC will work at the intersection of the legal services organizations and Zufall to facilitate referral, data sharing, collaboration, and program development.

Essential Functions, Duties, and Responsibilities

  • Provide comprehensive case management services that include referral and provision of direct one-to-one assistance to help clients obtain and consistently utilize legal counsel and legal assistance by MLP attorneys This will include assistance in completing applications, ensuring that questions are answered, and scheduling appointments, helping clients arrange transportation, providing follow-up reminders prior to scheduled appointments, and follow-up assistance in rescheduling missed appointments.

  • Carrying a caseload of at-risk clients and promoting the active engagement of patients with urgent legal issues, such as imminent eviction, working to identify and resolve issues as they arise.

  • Coordinator will participate with Zufall’s team to develop new processes for referral mechanisms and management of sharing of pertinent health information and health data in strict compliance with HIPAA (Health Insurance Portability and Accountability). Coordinator will participate with a working group in developing the forms, policies, and procedures for a viable, sustainable medical-legal partnership with MLPs.

  • CM will work in collaboration with Zufall providers and departments including Oral Health, Behavioral Health, and Special Populations.

  • CM will identify and cultivate formal linkages and informal partnerships with and become aware of community agencies and entities providing direct or related services as well as specialty care providers in order to maximize the resource engagement available in Zufall catchment areas.

  • CM will follow the HEAL and Florecer processes for determining program eligibility and will refer to and provide patients with linkages into respective MLPs.

  • Provide crisis intervention services as needed, focused on enhancing the client’s ability to problem solve as independently as possible, use effective coping skills, and manage their own care to the degree possible.

  • Work with Program Coordinator, MLP partners and other clinical staff using evidence-based practices in service delivery to implement the program, develop processes, track receipt of services and referrals, track required health outcomes, generate reports and conduct quality assurance and performance improvement activities

  • Participate as a key member of the Zufall PCMH and QA and clinical care team.

  • Participate actively in direct patient interaction and be responsible for intensive patient navigation to facilitate patient adherence to legal counsel and retention in care.

  • Coordinator will share responsibility for keeping project on track, according to published timeliness, and for documenting progress in Zufall’s electronic health record and according to grant funder agreements.

  • Participate in training, on and off site, offered by MLPs and other entities providing up-to-date education regarding relevant legal issues.

  • Ensure that progress notes and data entry on client contacts and collateral contacts occur no later than 5 days after the intervention, service or referral is provided. Specifically, ensure:

  • Data entry that is timely, accurate, confidential and entered in “real-time”

  • Data gathering process is part of the continuum of care as per Zufall’s Care Model and existing processes

  • Reports are generated and reviewed on a weekly basis to monitor program

  • Client tracking, reminders and call backs using Zufall’s electronic health record occur in a timely manner

  • Work collaboratively with Zufall’s IT and MLPs to share and integrate data, resolve issues and establish working processes

  • Assists in the development of monthly, semi-annual, annual and other required reports

  • Participates in health center quality assurance/performance improvement activities. Ensure that services are received by clients and that quality of care provided to clients is in keeping with the standards of the program, and the health center’s QA/PI plan/process

  • Work with grants and development team to pursue opportunities to enhance and sustain legal services at Zufall

  • Performs other duties as assigned by SVP of Medical Services and/or Chief Medical Officer.

    Knowledge, Skills, and Abilities

  • Excellent customer-focused interpersonal skills to interact in an effective manner with patients, practitioners, the interdisciplinary health care team, community agencies, and families with diverse opinions, values, and religious and cultural ideals

  • Excellent verbal and written communication skills

  • Bilingual in Spanish/English

  • Excellent computer skills, experience with electronic medical records a plus

  • Ability to interact with patients, staff and outside agencies appropriately

  • Ability to follow instructions, given either orally or in writing

  • Ability to work independently or with little supervision

  • Excellent organizational skills such as problem solving, decision making, priority setting, and work delegation

  • Ability to work well under pressure

  • Ability to complete assignments in a timely manner

  • Ability to work as a cooperative team member

  • Ability to maintain HIPAA compliance, corporate compliance and client confidentiality

    Education, Training, and Experience

  • Bachelor’s degree, or High School graduate with at minimum five years clinical experience

  • Bilingual in Spanish and English

  • Knowledge of Word and Excel required

  • Experience in the use of electronic medical records a plus

  • Experience in medical setting or some clinical background preferred

  • Self-starter and ability to work independently with minimal supervision

  • Excellent writing skills and organization abilities

    As a condition of employment, we require that all staff are fully vaccinated against flu.
