Overview

We are looking for an AVP Clinical for Advanced Illness Management to join our team. This position will directly report to the VP Palliative Clinical and is responsible for assisting in the development, implementation and evaluation of operational policies and procedures, processes and education.

  • Monitor and evaluate quality outcomes in their area of responsibility to ensure delivery of high-quality patient care and services

  • Ensure team members provide the appropriate resources and training to support the division in producing desired outcomes

  • Develops and maintains key partnership relationships, including but not limited to: value based physician partners, ACOs, physicians, and other healthcare entities if needed to support clinical delivery model

  • Assists in assuring that palliative Advanced Illness Management (AIM) meets and exceeds all applicable regulatory or state agency requirements, and assumes responsibility for the readiness of all palliative AIM location inspections, surveys, and review-including state licensure, federal certifications, and accreditations by implementing the company’s systems, policies and standards

About You

  • RN required, NP highly preferred

  • Experience in Hospice and/or Palliative Care, Oncology, Acute Care or Home health

  • Unencumbered and active RN license in state of residence as required by state for level of licensure, NP preferred

  • Certification if NP

  • 2 years previous managerial and/or educator experience in healthcare

  • Experience with Medicare FFS, ACO or value- based care delivery is preferred

  • Travel: approximately 75-80%
