Village Point is seeking Licensed Practical Nurses (LPNs) to join our team!
$2,000 Sign-On Bonus for part-time positions!
Village Point a Springpoint Senior Living Community located in Monroe Twp, NJ. At Springpoint, our LPNs are responsible for providing direct nursing care to the residents and to supervise the day-to-day nursing activities performed by the CNAs and CHHAs. Such supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be required by the Director of Nursing Services or Designee to ensure that the highest degree of quality care is always maintained.
Responsibilities:
Assume responsibility and accountability for the direct delivery of nursing services in accordance with established nursing standards and all SSL policies and procedures.
Provide medications and treatment to residents.
Assist with physician or nurse practitioner during rounds.
Make nursing rounds on the unit and report any problems to Unit Coordinator or supervisor immediately.
Notify the physician of any changes or concerns in the resident’s condition.
Respond to emergency calls throughout the community.
Supervise direct care given to residents by CNAs/CHHAs.
Educate and direct CNAs.
Complete all required documentation in an accurate and timely manner (e.g. nurses notes, Medicare A charting, GG assessments, Admission charting and observations, weekly wound assessments, psychotropic charting, etc.).
Ensure that the resident’s privacy, confidentiality, and dignity are always maintained.
Assist with evaluations/assessments of residents upon Admission and coordination of the residents’ plans of care, including baseline care plan.
Access and utilize portals for labs, diagnostics, and pharmacy to ensure prompt communication with physician for clinical resident management.
Complete signatures for controlled medication count sheet shift to shift.
Maintain the organization and cleanliness of the medication and treatment carts, and the medication room.
Education and Experience:
Highschool Diploma or equivalent required.
Must posses a current and valid NJ LPN license.
LTC/rehabilitation experience preferred.
