Work Schedules:

  • Monday-Friday, days

  • Work at home from anywhere in the US in the time zone you are located

As a Post-Acute Transition RN Case Manager, you will have the flexibility to work from home* as you take on some tough challenges.

Primary Responsibilities:

  • Completes post discharge assessments evaluating members for clinical and social needs timely and effectively

  • Follows model of care requirements as outlined in policies, procedures, and job aids

  • Educates members on disease process and medications to include completion of the Medication Reconciliation – Post Discharge

  • Evaluates members for ongoing needs and completes referrals as appropriate

  • Manages, communicates and updates plan of care with members

  • Collaborates with other programs and departments as necessary

  • Collaborates with Primary Care Physicians regarding medication concerns, questions, or other member needs

  • Ability to work independently and follow program processes

Required Qualifications:

  • Current, unrestricted RN compact license

  • 3+ years of experience in a hospital, acute care, or direct care setting

  • Basic level of experience with Microsoft Office (Word, Excel, Outlook)

  • Must have a designated workspace and access to install high-speed internet via hardwire in home

Preferred Qualifications:

  • Bachelor's Degree

  • Certified Case Manager (CCM)

  • Experience working in an outbound call center environment

  • Experience or exposure in discharge planning, utilization review, concurrent review, risk management or short-term case management.

  • Experience in a telephonic role

  • Solid organizational skills and multitasking abilities

  • Ability to work in a fast-paced environment

  • Bilingual, Spanish

  • Located in CST or MST

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
