University of Minnesota Physicians is seeking a Certified Nurse Practitioner or Certified Physician Assistant to join the Physical Medicine and Rehabilitation (PM&R) team at the M Health Fairview Acute Rehabilitation Center (ARC) at the University of Minnesota Medical Center. Practice Details: In this position, the successful candidate will work collaboratively with experienced APPs and academic PM&R physicians within a highly specialized rehabilitation care team. Our team includes Nursing (including Certified Rehabilitation nurses and Nursing Assistants); Physical Therapists; Occupational Therapists; Respiratory Therapists; Speech-Language Pathologists; Social Workers, Psychologists; Care Coordinators,consulting providers and medical trainees who collectively identify, develop, and implement an appropriate plan of care. Our population includes a wide variety of patients, most frequently treating patients recovering from stroke, brain injury, spinal cord injury, other neurological conditions, amputation, transplant, or several other surgical and medical diagnoses who require three hours of therapy daily up to 6 times per week, with an average length of stay between 7-21 days. The successful candidate will be responsible for delivering exceptional care to patients with complex medical conditions in our mission to support patients in their return to the community as freely and independently as possible. Key responsibilities of this position include: Monday - Friday, day shift 0.75 FTE with an expected work week consisting of approximately 30 hours Consulting on hospital inpatients evaluating appropriateness for next level care including acute rehab setting Performing daily rounds, admissions, discharges and consults and completing the proper documentation Daily collaboration with physicians,physical therapists, occupational therapists, speech therapists,psychologists, care coordinators, dietitians, pharmacists, social workers, other consulting providers and medical trainees to ensure provision of the highest quality and efficient patient care.
