Minnesota Visiting Nurse Agency Mental Health Professional - Licensed in Minneapolis, Minnesota

SUMMARY:

We are currently seeking a Mental Health Professional – Licensedto join our team. This full time role will work days.* This is a grant funded role, currently funded through March 2025 with potential for extension.*

Purpose of this position:Under general supervision, a mental health professional provides direct mental health services including screening, diagnostic assessment, psychoeducation, and psychotherapy services with persons living with mental illness; evaluates care and programming; and develops programming as needed

RESPONSIBILITIES:

  • Participates in multidisciplinary care team rounds
  • Provides mental health services including initial and ongoing screening, diagnostic assessment, treatment planning (including regular updates), psychoeducation, psychotherapy services, crisis services, referrals, and discharge/termination planning
  • Completes necessary documentation for accurate and timely billing of clinical services provided
  • Provides psychoeducation to patients, families, and community organizations as needed
  • Provides care coordination including discharge/termination planning
  • Provides on-call phone coverage for patient coaching as required
  • Provides clinical supervision to graduate-level mental health practitioners in compliance with HHS and regulatory guidelines (e.g., licensing boards, state regulations, etc.)
  • Assists in maintaining standards of accreditation by health management organizations and state licensure/certification
  • Other duties as assigned

Positive Care Ryan White department specific:

  • Primary therapist for walk-in therapy
  • Go-to for staff who have questions about therapy/mental health
  • Be part of the “new patient orientation” to talk about MH services available in clinic/in the community if patient expresses an urgent need and wants to talk
  • Work with psychotherapists to support patients needing to get into a crisis residence or IRTS program
  • Connecting patients to other therapists in the community and MH resources (i.e. patients want a therapist who speaks other languages, interest in a therapist that has specific identities, information about how to get referred to ARMHS etc..)
  • Connecting patients with community programs and resources that would be beneficial for mental health wellbeing and building support system (i.e. Clues, Aliveness, NACC etc.)

QUALIFICATIONS: /**/

/Minimum Qualifications:/

  • Meets criteria for “Mental Health Professional” as defined in MN statute
  • Master’s degree in a behavioral health field

/Preferred Qualifications:/

  • Experience providing psychotherapy groups and experience working with persons living with mental illness and addiction

/Knowledge/ Skills/ Abilities:/

  • Ability to counsel and instruct patients and families
  • Ability to recognize and document patient progress
  • Ability to work cooperatively with other members of the multidisciplinary clinical team
  • Ability to interact effectively with individuals from diverse backgrounds including ability to deal objectively with individuals whose attitudes and behaviors differ from her/his own
  • Ability to read and interpret written instructions, patient charts, and physicians' notes
  • Ability to interpret policies and procedures in an advisory capacity
  • Ability to use computer systems, including electronic medical records, along with working knowledge of Microsoft Office Products (Outlook, etc.)

/License/Certifications:/

  • Must be a licensed Mental Health Professional per MN statute – LMFT, LICSW, or LPCC
