We are currently hiring for the Manager, System Clinical Integration role . This leadership role maintains the day-to-day clinical, operational and financial structures and processes that provide direct support to and delivery of Integrated Behavioral Health (IBH) and Behavioral Health Home (BHH) services across Fairview Health Network including Fairview Primary Care Service Line, Fairview Physician Associates, University of Minnesota Physicians, the Clinics and Surgery Center and departments across Fairview Health Services.

Responsibilities include the design, development, delivery and ongoing management of IBH / BHH services within and across multiple entities and healthcare systems assuring consistent and coordinated application of standardized models of care delivery. Clinical and operational support extends to outpatient clinical services that interface with medical sub-specialty services. Responsible for achieving performance metrics to deliver system goals for quality, employee engagement, total cost of care, and population management.

Job Expectations :

  • Advancesthe design, development, implementation of IBH / BHH care structures and processes within both primary and specialty medical services across the system in coordination and collaboration with behavioral, medical and academic leadership

  • Insures integrated care delivery models are supported with implementation and training plans.

  • In collaboration with system partners and services, responsible for coordinating and conducting key hiring components of IBH / BHH staff including partnering with and supporting recruitment activities; conducting initial rounds of interviewsfacilitatingcandidate selectionand selecting final round candidates for medical team interview panels to participate in the final selection and hiringprocess.

  • Responsible for the coordination and provision of new IBH / BHH staff onboarding, orientation, training, ongoing clinical supervision andperformance management.

  • Responsible for working directly with medical leadership and providers to orient and support primary and specialty care teams in restructuring and adapting care delivery processes to enable IBH / BHH care delivery.

  • Provides support to Behavioral Health Clinicians (BHC), IBH team members,MHFC and Health Psychology clinicians, nursing and rooming staff, and leadership addressing urgent and routine clinical and operational needs.

  • Provides ongoing consultation and collaboration to primary and specialty care teams to sustain effective integrated care delivery services

  • Meets with care team and providers on a routine basis to provide clinical and operational direction, oversight and support.

  • In collaboration with IT supports development, maintenance and application of electronic data systems and dashboards to meet program requirements, inform care delivery, and performance.

  • Works in collaboration with IT support to optimize epic based applications in support of program needs and goals.

  • Achieves performance metrics to support system goals for quality, employee engagement, total cost of care, and population management.

  • Creates and maintains highly effective partnerships

  • Collaborates with system leadership and other partners to develop and execute Behavioral Services short, mid and long term goals to ensure improved access and strong financial performance.

  • Represents behavioral services in committees and leadership groups across the system to advance integration and partnership across medical and behavioral health systems and services

  • Maintains knowledge base of all regulatory and reporting requirements for programs and ensures compliance for all requirements issued by regulatory agencies.

  • Establishes and maintainsappropriate relationships with agency representatives to achieve program requirements and objectives

Qualifications

Required

Education

  • Registered Nurse Experience

  • Minimum of three years’ experience in administrative, clinical or healthcare management and organizational development, or equivalent.

License/Certification/Registration

  • Licensed as a registered nurse in the state of MN

Preferred

Education

  • Advanced Practice Nursing

Experience

  • Seven or more years post licensure experience

  • At least five years’ experience in clinical supervision

  • Five to ten years’ experience supervising mental health services in outpatient settings. Prefer experience supervising multiple programs and/or locations. Prefer experience gained in large, complex systems. Experience integrating mental health care within a medical setting highly preferred.
