Minnesota Visiting Nurse Agency Hospital Responder Supervisor - Next Step in Minneapolis, Minnesota

The Next Step Program is a hospital based program that works with individuals who are victims of gun and/or violent crimes. The program provides comprehensive case management services that include employment, education, and trauma support to program clients and their family members.

SUMMARY

Provides supervision of HHS Hospital Responders staff who have responsibility to respond to Hennepin Healthcare, North Memorial and Abbott Northwestern emergencies. This includes ongoing individual and/or group supervisory meetings, ongoing feedback regarding job performance, participation in case review conferencing and service planning, evaluation of paperwork and documentation, training, and evaluation.

Schedules for appropriate coverage to fulfill the business need which includes both normally scheduled shifts as well as on call and emergency coverage. Ensures staff are appropriately trained in the areas of de-escalation, violence prevention, confidentiality. Addresses sensitive concerns on matters that need to be elevated beyond the Hospital Responder.

RESPONSIBILITIES

  • Provides leadership to staff, promotes a sense of agency with co-workers and staff, and communicates in ways that support agency mission and values
  • Ensure service delivery and documentation compliance in accordance with Hennepin Healthcare, North Memorial and Abbott Northwestern standards and written agreements and contracts
  • Coordinates program activities with appropriate liaison agencies to ensure constructive collaboration
  • Provides direct service as a Hospital Responder
  • Provides program service and administrative supervision of program staff, students, and volunteers
  • Assures the continuing training and development of program staff
  • Regularly schedules meetings with program staff to discuss plans, address areas of potential problems and program functioning
  • Develops and promotes relationships with other social service agencies and represents the program and agency in the community
  • Performs administrative functions including the completion of regular reports in a timely manner as required by funding sources and the Agency
  • Participates in agency collaboration team meetings
  • Contributes on Agency committees and special tasks and communicates in ways that support agency mission and value.
  • Adhere to policies and procedures of Agency Documentation of all participant contacts and activities
  • Arrive at hospital within 30 minutes of notification; once activated, stay at hospital as needed or until shift ends
  • Conduct initial bedside visit(s)
  • Provide immediate crisis response for patient and family support system
  • Provide comfort/education about emotional consequences following violent injury
  • Build rapport, assess risk for retaliatory violence, assess strengths and needs, and determine need for community-based follow-up services
  • Link participant to Next Step Case Manager when appropriate
  • Documentation (participant roster, participant contact, necessary follow-up)

QUALIFICATIONS

Minimum Education/ Work Experience

  • Bachelor’s Degree in criminal justice, mental health or related field
  • A minimum of 6 years of working in a related field with 2 years of supervising employees

OR

  • An approved equivalent combination of education and experience
