Job Summary: The responsibility of the Case Manager is to assist 6-12 clients in their transition from incarceration into the community as well as build and incorporate new relationships with community organizations. Assist Program coordinator and Program Manager and all other staff in the operation and follow through of services, program requirements and licensing. This includes organizing volunteers to help with these responsibilities. The Case Manager is also responsible for daily supervision and training of volunteers, as well as delegating tasks to volunteers. Additional Responsibilities Include: Account for all residents by performing headcounts, phone check in, keeping sign in/out records and having knowledge of residents' whereabouts. Identify resident's whereabouts at the beginning of each shift. Meet with clients and case note relevant information on a weekly basis minimum. Complete assessments and make appropriate referrals to evaluations, testing or licensed assessors. Develop a comprehensive referral follow up system to maintain a contiguous path of services. Maintain communication with outside agencies involved in client transition. Complete monthly/weekly progress reports as required. Complete and submit all required reports to the client's supervising agent within time requirements. Build new relationships with community organizations and employers. Maintain order and security in the house by monitoring residents, screening visitors and enforcing regulations. Qualifications and Requirements: Bachelor's degree in Social Service or 3 years of working with clients, life skills training, and group facilitation. A valid driver's license, acceptable driving record, reliable transportation, valid insurance, and a Motor Vehicle Check. Strong organizational skills. Ability to be focused and attentive for an extended period of time. Able to develop, guide, and provide follow-up for client transition plans. Ability to promote a positive attitude regarding LSS and other programs to the public. Knowledgeable regarding community programs and referrals.
