Salary $44,798.65 - $64,757.87 Annually Job Type Full-time Job Number 24HSPH050JSS056 Closing Date 9/16/2024 11:59 PM Central Description The Annual Assessment (AA) team in the Long-Term Services and Supports (LTSS) Service Area of the Hennepin County Health and Human Services (HHS) department, has an opening for a Case Management Assistant (CMA). The Assessment teams administer Medicaid waiver and state programs that serve seniors and individuals with disabilities including brain injury, mental health, complex medical, developmental, and physical disabilities. The applicant for this position will use a person-centered approach to work across interdisciplinary teams of Public Health Nurses (PHN) and Social Workers (SW) to follow-up with individuals assessed for Long-Term Services and Supports. Location and hours: This position is hybrid and will be performed both on-site at 6625 Lyndale Avenue South, Suite 200, Richfield, MN 55423 and Hennepin County Government Center, 300 South 6th Street Minneapolis, MN 55487 and remote as job duties require. Travel to community service providers and individuals' homes within Hennepin County and Hennepin County Service Centers will be required. This position will work Monday-Friday from 8 a.m. to 4:30 p.m., evenings and weekends may be required. While this position is designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin. New employees who are hired into remote or hybrid positions between January 2, 2022 to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment. In this position, you will: Work in collaboration with external collaterals and internal business partners to acquire and update or complete documents needed to ensure continued eligibility, and payment for and continuation of services. Follow up with persons served to gather information and provide assistance for completion of county and state documents needed to ensure continued eligibility. Manage and maintain person related records or other information records, including collecting documentation for continued eligibility. Conduct file review and obtain person information from multiple county and state data bases. Navigate in database systems such as SSIS, ECF, and MMIS as well as able to manage data in Excel, and other Microsoft applications and/or other reporting and managing systems. Assist with case assignments and initiate case closures per request/as delegated by the supervisor. Maintain and organize data for reports for assisting with managing workflows, including tracking demographic data and ongoing service and payment data. Participate in team s effort to ensure processes/protocols are in compliance and meeting outcomes. Stay informed/current with relevant state statutes, policy, and county procedures.
