Reintegration Specialist

Key Responsibilities

The Reintegration Specialist with work with the Director of Mental Health Services to support transitioning individuals from hospital settings into the community. Responsibilities of this position include but are not limited to creating unique and creative discharge plans for individuals who require a high level of care. This position will review referrals, meet individuals and create person centered plans, determine level of service needed, secure housing, work with providers, work directly with individuals, and obtain other resources identified for individuals needing supports. This position will require travel and work with other service providers throughout the state.

Requirements

  • 18 years of age or older.

  • A bachelor’s degree is required in one of the behavioral health or allied fields (such as, but not limited to; criminal justice, mental health, special education, or social work) from an accredited college or university

  • A master’s degree is preferred in one of the behavioral health or allied fields (such as, but not limited to; criminal justice, mental health, special education, or social work) from an accredited college or university

  • A minimum of three years of related work experience within the human services industry with an emphasis in working with SPMI.

  • Skilled in written and oral communication, community relations and service development.

  • Demonstrated knowledge and understanding of behavior management principles, conducting assessments, writing plans and providing recommendations.

  • Eligible as determined by the Bureau of Criminal Apprehension, the Department of Human Services, and the Company’s Corporate Compliance Plan.

  • Valid driver’s license and driving record that meets driving record requirements as defined by the Company.

  • This position will be based at WFH but requires regular travel around the state of MN.
