Location: Middletown, NY

Rate of Pay: $20.29 Hourly

Hours:Monday -Friday 9am - 5:30 with flexibility to be available at least 3 evenings per week

NYS Certified Peer Specialist/NYS Certified Youth Peer Advocate or be able to obtain at least one of these certifications.

Seeking a Transitional Care Coordinator with lived experience with a mental health challenge s and is willing a nd able to share that experience . The Transitional Care Coordinator wil l work with young adults ages 16 -25.

Job Description:

In this role you will:

  • Attend SPOA Meetings

  • Share your lived experiences to support the young adult in the program

  • Provide services in the form of direct support, information sharing, and skill building.

  • Coach, educate, support, encourage young adult to use their voice and make informed choices.

  • Increase young adult self-empowerment and advocacy skills.

  • Mediate, facilitate, or bridge between young adult and agencies.

  • Assist young adult in:

    • Identifying natural supports

    • Connecting to informal supports

    • Discovering their own strengths

    • Ensuring the ir plan is customized and individualized

    • Defining what success looks like the young adult

    • Resolving conflicts

    • Integrating into community, school, and family

    • Achieving successful independent living options

    • Obtaining employment

    • Transitioning into adulthood

    • Navigating the public human services system

    Qualifications:

    The ideal candidate will have:

  • Have a working knowledge of the MH, education, social, employment, and community services

  • Must be able to relate to issues faces by young adults, while supporting and providing a positive attitude

  • Demonstrated direct experience with their own mental health challenges

  • Ability to work with a variety of agencies/organizations/schools

  • Must be organized and able to prioritize

  • Have good organization , writing and telephone skills

  • Willingness to participate in ongoing training s

  • Report to the Senior Family Partner

  • NYS Certified Peer Specialist /NYS Certified Youth Peer Advocate or be able to obtain at least one of these certification s

    • Must have dependable transportation
