Unity House Assistant Program Director in Menands, New York

Position is eligible for a $750 sign on bonus.

Responsibilities:

  • Supports the AccesVR Program Director,

  • Navigation of the AccesVR contract to ensure timely submission and coordination of all referrals.

  • Submission or all corresponding documentation of AccesVR forms.

  • Reviews AccesVR referrals for eligibility and acceptance.

  • Follows up with staff regarding updating and completing correct documentation and forms.

  • Provides training to staff on an on-going basis with all aspects of paperwork for both Intensive and Extended consumers.

  • Updates AccesVR calendar to remind staff of milestone due dates, team meeting date/time/location.

  • Work with other to complete projects and reports.

  • Promote an environment of inclusion and belonging.

    Requirements:

  • Bachelors Degree in Human Services or related field; or Associates Degree plus Human Services experience.

  • Minimum of 1 year of experience with individuals with disabilities, as Case Manager or Vocational Counselor preferred.

  • Proficient with Microsoft office, Onenote, excel.

  • Clean / Valid NYS Driver’s License required

  • Good customer service skills.
