DUTIES & RESPONSIBILITIES
ESSENTIAL FUNCTIONS:
Competently completes or coordinates the completion of therapy evaluations/screens upon physician orders, making appropriate recommendations for nursing regarding mobility, positioning and range of motion.
Able to coordinate with therapists to establish and provide therapeutic interventions per residents’ plan of care and per physician order.
Ability to modify treatment program levels, intensity and techniques in conjunction with treating therapists’ input based on the residents’ response to specific treatment plans.
RISK MANAGEMENT:
Applies safety principles and precautions by providing a safe and secure environment for residents, significant others, staff and self through the application of safety principles and precautions.
Demonstrates awareness of and sensitivity to residents’ rights. Maintains dignity and confidentiality of the resident and serves as a resident advocate—ensures Rehabilitation Department staff does the same.
Performs efficiently in emergency situations; remains calm, uses good judgement, follows established protocols, informs appropriate personnel and documents events if required.
Performs within scope of practice, demonstrate awareness of legal issues, utilizes judgement and actions to reduce potential adverse consequences for resident, institution and self.
Reviews all therapeutic documentation for completeness and accuracy.
ACCOUNTABILITY/GOALS:
Assumes responsibility for own practices which fall within the Physical Therapist’s scope of practice, facility standards, policies and procedures, and ensures all Rehabilitation Department staff does the same.
Supports the philosophy and mission of Highland Rehabilitation & Nursing Center and participates in the development and attainment of department goals.
SELF-DEVELOPMENT/SELF VALIDATION:
Identifies personal learning needs and seeks opportunities for learning; serves as a clinical resource for Rehabilitation and Nursing staff.
Conducts department-related conferences and in-service programs to identify and/or discuss issues and subsequent interventions.
Validates knowledge base, skill level and decision-making abilities by seeking guidance from other members of the health care team when appropriate.
Participates actively in staff development activities for department and provides support and guidance to colleagues.
QUALITY IMPROVEMENT:
Communicates all pertinent information (clinical findings, problems, concerns, ideas) in a timely, effective manner using appropriate lines of authority.
Accept responsibility for own conduct and the conduct of the Rehabilitation staff.
Demonstrates ability to cooperate with and assist fellow health team members.
Remain flexible in cooperation with scheduling and staffing needs for the provision of patient care, including temporary assignment as necessary.
Accept responsibility in complying with the facility attendance/punctuality standard.
Participates in and identifies quality improvement initiatives.
OTHER PERFORMANCE FACTORS:
Does not abuse or take advantage of sick time or personal days.
Consistently reports to work on time and ready to work at the start of the shift.
Always provides proper notification when late or absent.
Demonstrates the ability to recognize priorities in organization of work flow. Able to perform duties independently, with minimal need for direct supervision.
Maintains the confidentiality of all residents, consultants, and employee related information.
Reports to work well-groomed and in compliance with Highland Rehabilitation & Nursing Center’s dress code. Wears ID badge while on duty.
Demonstrates the ability to interact in a positive and helpful manner with residents, visitors, physicians, volunteers and co-workers.
