JOB SUMMARY

The Registered Nurse-Hospital functions as a vital member of the healthcare team by demonstrating a customer/patient focus, effective communication, professionalism and self-development, and teamwork. The individual utilizes the nursing process to assess, plan, implement and evaluate comprehensive care provided to selected patient populations and their families. The Registered Nurse - Hospital is expected to demonstrate excellent clinical, problem-solving, interpersonal and leadership skills and make decisions consistent with hospital policies, department protocols/guidelines, and the Wisconsin Nurse Practice Act.

The staff of the 16-bed MICU provides 24-hour nursing care to critically ill adult/geriatric patients >18 years of age with a variety of medical conditions and strives to meet the unique cultural and developmental needs of the patients. MICU may serve as an overflow unit for SICU or PICU in the event either unit is full and unable to triage.

This position will work in the Marshfield Medical Center, Marshfield, WI in the Medical Intensive Care Unit.

WORK SHIFT:

Baylor/weekend Schedule: This position will work 36 hours every Friday, Saturday and Sunday and be paid an additional 18 hours of Baylor pay. Employees are required to work 36 hours from Friday at 6am to Monday at 8am in the Weekend/Baylor program.

JOB QUALIFICATIONS

EDUCATION

(For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation.)

Required Education: Diploma or degree in nursing.

Preferred Education: Bachelor's degree in nursing.

EXPERIENCE

Preferred/Optional Experience: Experience in an acute care setting.

CERTIFICATIONS/LICENSES

The following licensure(s), certification(s), registration(s), etc., are required for this position, which must be maintained in good standing:

Minimum Required: Current State of Wisconsin Registered Nurse License or Nurse Compact License. Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) within three months of hire. Advanced Cardiac Life Support (ACLS) certification on hire or during orientation.

Medical Intensive Care Unit [MICU]

Registered Nurse - Hospital

DEPARTMENT/UNIT DESCRIPTION

The staff of the 16-bed MICU provides 24-hour nursing care to critically ill adult/geriatric patients >18 years of age with a variety of medical conditions and strives to meet the unique cultural and developmental needs of the patients. MICU may serve as an overflow unit for SICU or PICU in the event either unit is full and unable to triage.

Services Provided/Patient Conditions:

Conditions treated in the MICU include:

  • Myocardial infarction

  • Acute coronary syndrome

  • Cardiomyopathy

  • Congestive heart failure

  • Dysrhythmias

  • Pulmonary Emboli

  • Deep Vein Thrombosis

  • Interventional radiology procedures (i.e., PTCAs, stents, angiogram, coiling, IA tPA, thrombolectomies)

  • Cerebral Vascular Accidents

  • Subarachnoid Hemorrhage

  • Intracranial Hemorrhage

  • Respiratory Failure

  • Pneumonia

  • COPD

  • Sepsis

  • Cardiogenic/septic shock

  • GI Bleed

  • Acute and Chronic Renal Failure

  • Liver failure

  • Pancreatitis

  • Drug Overdose

  • Seizure

  • Multiple Organ Failure

  • Acute Respiratory Distress Syndrome (ARDS)

Staffing Mix:

The MICU Unit is staffed with registered nurses and is supported with nursing assistants and health unit coordinators. Nurse to patient ratio is based on acuity and is usually 1:2 or 1:1. Registered Nurses function as the primary care giver or as direct supervisors to the care provided by nursing assistants.

Registered nurses complete a 10-12 week clinical and didactic critical care orientation addressing general critical care practices, as well as unit specific practices, or, they have completed the MMC Critical Care Residency Program. Specialty orientations to address acute high risk patients and populations and unit required certifications are completed within the first year of employment in MICU.

Registered nurses are the primary caregivers in MICU. Other team members of the MICU include: physicians, physician assistants, nurse clinicians, pharmacists, certified nursing assistants, respiratory therapists, physical, occupational, and speech therapists, as well as individuals from the Spiritual Services Department, Metabolic Support Team and Dietary, Patient Care Advocate, Case Management and Social Work, and Cardiac Rehabilitation.

Additional resources may be used to achieve the most appropriate multidisciplinary team for care of the patient and family. Nursing practice and delivery of care is provided in accordance with the Critical Care Standards of Practice and American Association of Critical Care Nurses guidelines.

Patient Population:

Critically ill adult/geriatric patients >18 years of age with a variety of medical conditions.

Frequent Procedures & Equipment Used:

Intensive care services include the following therapies:

  • Simple and complex dysrhythmia interpretation and intervention

  • Hemodynamic stability monitoring and intervention

  • Airway maintenance/oxygenation/ventilatory supports needs

  • Delivery of PCA/sedation with BIS monitoring as needed

  • Peripheral nerve stimulation monitoring

  • Medication administration (including vasoactive drug administration and titration)

  • Blood product administration

  • Specialty bed operation

  • Nutritional support (parenteral and enteral)

  • Lab value monitoring

  • Wound/skin/tube/drain care

  • Chest tube maintenance/monitoring

  • Pulmonary artery monitoring

  • Intra-aortic balloon pump monitoring

  • Pacemaker monitoring (temporary transvenous or transcutaneous and permanent)

  • Thermoregulation via blood/body cooling/warming/transfusers

  • Continuous renal replacement therapy

  • Percutaneous LVAD/Impella monitoring

  • Therapeutic hypothermia/normothermia monitoring

  • Continuous EEG monitoring

  • Proning

SERVICE LINE/DEPARTMENT LICENSES, REGISTRATIONS OR CERTIFICATIONS

Individual must meet the minimum qualifications as stated within the system job description. The items noted below are in addition to the minimum qualifications.

Minimum Required:

  • Advanced Cardiac Life Support (ACLS) certification on hire or during orientation.
