Area Agency on Aging Adult Protective Services Case Manager in Marion, Ohio

Requirements:

  • Education: Associate's degree in human services or related field, OR commensurate experience OR Bachelor's degree from accredited college or university in Social Work, Psychology, Sociology, Gerontology, Nursing, Health Science, Health Care administration, Human Resources, Political Science, Anthropology, Human Services, Public Administration, Education, Counseling, Criminal Justice or a closely related field
  • Available for after-hours calls
  • Strong problem-solving skills
  • Strong written and oral communication skills
  • Ability to communicate professionally, respectfully and effectively
  • Ability to travel in a multi county service area and access private homes and facilities, which may not be readily accessible

Duties include, but not limited to:

  • Intake screening and report processing of calls alleging abuse, neglect, and/or exploitation, including after-hours calls
  • Inputs intake screening data and other data as necessary into appropriate documentation and data collection software, as needed
  • Receives and directs incoming calls and other correspondence as needed
  • Investigates, as directed, reports of abuse, neglect and/or exploitation; conducts home visits for observation
  • Assesses living arrangements, physical capabilities, cognitive abilities, medical requirements and financial resources
  • Develops protective service plans and provides on-going follow-up and monitoring to reduce risk and increase safety of reported adults

  • Recommends, prepares and attends legal interventions
  • Provides information and referrals to local agencies and organizations and coordinates community resources
  • Completes case documentation using appropriate computer applications to document events, record interviews and other relevant information
  • Responds to reported elder-related emergency situations outside normal agency operating hours
  • Meets procedural deadlines governing management of APS

Job Type: Full-time

Pay: $19.54 per hour
