University of Minnesota Physicians is seeking a Certified Nurse Practitioner or a Certified Physician Assistant who will deliver care for patients on the University of Minnesota Heart Care team at M Health Fairview Heart Care Clinic in Maplewood, MN. The position will provide patients with expert patient care and coordination of services. They will assist the practice to improve patient satisfaction and enhance the quality of care provided to patients through increased access and coordinated services. In This Role, You Will: Join a multidisciplinary team of 22 Cardiologists, 1 HF focused, and 12 Advanced Practice Providers The PA or NP provides and coordinates care for adults with cardiac diseases throughout the course of evaluation and treatment. Provide outpatient management for adults with heart failure of various etiologies, with potential forrounding and inpatient management of heart failure patients Provide patient and family education as it relates to heart failure and general cardiology. Provide clinic coverage in heart failure and general cardiology. Performs clinic follow up visits and potentially helps with inpatient consults. This individual will show commitment to exemplary patient care. Manages patient care through appropriate use of available resources. Demonstrates ability to deliver care or service adjusting approaches to reflect developmental level of population served.
