RN Care Manager

Job Description

RN Care Manager is responsible for clinical assessment, member-centered care plan development, ongoing care coordination, quality assurance, and monitoring of services to program members, who may be older adults and/or individuals who have intellectual or physical disabilities.

  • Achieve member outcomes and goals through adherence to standards of care and best practices

  • Interact with members and their families with the highest level of respect, compassion, and integrity

  • Identify and/or participate in efforts to continuously improve the quality of member care and services

  • Foster a member-centered culture on a daily basis

  • Participate in efforts to enhance work experiences

  • Actively seek and participate in training and development opportunities

  • Support the achievement of operational goals by adhering to operational standards and complying with contractual, regulatory, and CCI policy requirements and expectations

  • Complete discipline-specific initial and ongoing comprehensive assessments

  • Conduct home visits and home assessments as needed, and consult with families and other supports for additional information as needed

  • Use critical thinking to optimize members' health and quality of life to reach goals

  • Develop and maintain care plans

  • Educate members and families

Hard Skills

  • 2 years of Care/Case Management experience

  • Licensed WI LPN or RN

  • Creating Care Plans

Work Environment

Manage schedule based on members' needs. Primarily day shift hours 8am to 4:30pm, Monday through Friday. Required to do in-home and/or LTC Facility visits with members. Option to work from home but access to an office for training and internal meetings. Training primarily in the office for the first 3-6 months. Ongoing expectation to be in the office generally 3 days per week.
