The Case Manager II assesses client needs, abilities and strengths; facilitates development of client case plans and monitoring to determine progress both individually and as a program. Networks with other agencies to identify and collaborate with resource development and delivery. About the role: Conducts casework interviews with clients to obtain information for formulating program/service eligibility and case status. Identifies psychosocial, economic, and physical needs of clients; assesses and reassesses client\'s support systems, available community resources and other factors to plan, develop, and implement an appropriate service plan. Assist client with housing search, placement, and stability activities. Develops and implements a case plan that presents intervention strategies to meet client. Refers clients to appropriate community resources and advocates for or assists the client in obtaining such services. Prepares complete and accurate case notes; writes correspondence, reports, and other written materials; inputs data into HMIS. Reviews all assigned cases on a scheduled basis to determine continuing need for services and participate in case consultations. Provides training to new case managers. Regularly peer review and audit of case files to ensure of services standards are met and contract compliance. Assists in transporting clients and advocating for them with public and private agencies. Attends meetings with other community service providers on behalf of The Salvation Army, as assigned.
