McLaren Health Care Nurse Navigator - Oncology - McLaren Careers in Macomb, Michigan

Position Summary:

Provides direct patient care and comprehensive coordination of care for patients and their families/caregivers. Eliminates barriers to timely care, facilitates flow through system, facilitates interactions with clinical research office, increases patient and provider satisfaction, maintains point of contact with providers and serves as the first point of contact for patients and families. Provides a full range of nursing services, including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with professional practices and ethical standards.

Essential Functions and Responsibilities:

  1. Provides direct patient nursing care for specified patients [may include wound care, ostomy care, accessing port/PICC lines, appropriate supportive care and administration of chemotherapy, blood components, fluid and electrolyte replacements, and other oncology treatments as prescribed].

  2. Performs nursing assessments and triage of patient care needs for new and ongoing patients, including patients on a clinical research study.

  3. Oversees the planning, scheduling, and implementation of day-to-day clinical activities and procedures for navigated patients.

  4. Works with physician and clinical research office to facilitate, coordinate and ensure patient access to clinical trials where appropriate.

  5. Central point of contact for all navigated patients; including interaction with medical, nursing, ancillary services, and when appropriate the clinical research services.

  6. Provides education to patients, families, and significant others; acts as an information resource to students, health care professionals, patients, and the public.

  7. Collects and reports data from patient on barriers, health care, medications, prior treatments, etc.

  8. Tracks order completion and follow up. Monitors patient compliance with patient treatment plan.

  9. Assists patients with referrals to outside resources and care coordination as needed.

  10. Designs or participates in the creation of health education materials.

  11. Provides and documents quality, personalized and evidence-based care and interventions.

  12. Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies. May develop and conduct in-service training programs.

  13. Complies with federal, state, and local legal and certification requirements by studying existing and new legislation; anticipating future legislation; enforcing adherence to requirements; advising management on needed actions. This includes compliance with all health care regulations, such as HIPAA and OSHA.

  14. Performs other related duties as required and directed.

Qualifications:

Required:

  • Nursing degree from an accredited educational institution.

  • State licensure as a registered nurse.

  • Three years of experience directly related to the duties and responsibilities specified.

  • BLS certification.

Preferred:

  • Bachelor's degree in nursing.

  • One year prior work experience in clinical area.
