Summary of Primary Function

The Community Health Registered Nurse (RN) plays a crucial role in improving the health and well-being of the community by providing comprehensive healthcare services. This role involves direct client care, health education, community outreach activities, and addressing Social Determinants of Health (SDOH). The Community Health RN will work collaboratively with other healthcare professionals, community organizations, and individuals to promote and maintain the health of the community.

Essential Job Functions

  • Conduct comprehensive health assessments and screenings.

  • Develop and implement individualized care plans.

  • Administer medications, treatments, and vaccinations.

  • Educate clients and families about health conditions, treatments, and preventive care.

  • Conduct community health education workshops and seminars.

  • Offer counseling on lifestyle changes to improve health outcomes.

  • Identify health needs within the community and develop programs to address them.

  • Conduct SDOH screenings to identify factors such as housing, food security, education, and employment that may impact health outcomes.

  • Make referrals to appropriate community resources and support services.

  • Follow up with clients to ensure they receive the necessary support and services.

  • Collaborate with community organizations and agencies to coordinate services and resources.

  • Participate in community events and health fairs to promote health awareness.

  • Coordinate care for clients with chronic illnesses or complex health needs.

  • Assist clients in navigating the healthcare system and accessing resources.

  • Monitor client progress and adjust care plans as needed.

  • Maintain accurate and up-to-date client records.

  • Prepare reports on community health activities and outcomes.

  • Ensure compliance with regulatory and organizational standards.

  • Stay current with best practices and advancements in community health nursing.

  • Participate in continuing education and training programs.

  • Contribute to quality improvement initiatives within the department.

  • Proficiency in electronic health records (EHR) and Microsoft Office Suite.

  • Knowledge of public health principles and practices.

  • Possession of a personal automobile, current Ohio driver's license, and automobile insurance with $100,000/$300,000 liability required.

  • Ability to work evenings or weekends as needed.

Certifications

Current nursing license in the State of Ohio and BLS, ACLS, PALS and any other unit specific certification requirement

Education

Bachelors in Nursing (required)

Maters of Science, nursing (preferred)
