Job Title: RN Case Manager

Location: Long Beach, CA (Hybrid Role)

Schedule: Monday to Friday, 8:00 AM - 5:00 PM

No weekends, no on-calls

Position Overview:

We are seeking an experienced and compassionate RN Case Manager to join our small but growing team. This hybrid role is primarily virtual, with occasional fieldwork, particularly in the Long Beach and greater LA/OC area. As an RN Case Manager, you will be instrumental in coordinating care for Medicare patients, many of whom are aged 65 and over. Your work will directly impact the lives of underserved populations, and your efforts will help prevent complications from preventable health-related causes.

Key Responsibilities:

  • Care Management: Engage with patients virtually and in the field to manage and coordinate their care. Support transitions of care, including post-discharge follow-ups with partnering providers.

  • Census Management: Review daily censuses, manage inpatient, ER, and skilled nursing facility stays. Identify patients at risk and engage them in care programs.

  • Outreach & Collaboration: Perform outreach to hospital case managers and collaborate with various partners, including physicians, social workers, and other team members. Build and maintain strong professional relationships to support patient engagement.

  • Patient Interaction: Engage higher-risk patients, particularly those within the ACO program, to enroll them in high-risk home programs. Telephonic outreach to patients may be required.

Qualifications:

  • Experience: At least 5 years of relevant clinical experience, with a minimum of 3 years in care management within a health plan, home health, or hospice setting.

  • Background: Strong case management experience is required.

  • Education: Bachelor of Science in Nursing (BSN) required. Active CA RN license is mandatory.

  • Flexibility: Ability to adapt to a dynamic, evolving environment. This is a young organization where processes may change frequently, and flexibility is essential.

  • Mission-Driven: A genuine desire to give back to underserved populations, with a commitment to helping prevent health-related complications and deaths.

  • Self-Motivated: Ability to work independently with minimal oversight, while still being a collaborative team player.

Additional Information:

  • Work Environment: Hybrid role, with more virtual work than fieldwork. Company laptop and mileage reimbursement provided.

  • Team Structure: Small team including a physician, social worker, and per diem NP. The case load will start small but may grow as the program develops.

Pay Details: $85,000.00 to $90,000.00 per year
