We are seeking a skilled RN Case Manager who thrives in a fast-paced environment and utilizes clinical expertise, discretion, and independent judgment to perform the nursing process, including assessment, care planning, intervention, and evaluation within a patient-centered care professional practice model (Duffy’s Quality-Caring Model). The role involves specialized case and utilization management methodologies and practices, ensuring medical appropriateness criteria (medical necessity) are met for the level of care provided, documented from admission through discharge. The RN Case Manager will practice in a collaborative healthcare environment, overseeing the implementation of a well-thought-out interdisciplinary plan of care, including an individualized discharge plan that comprehensively meets the continuing healthcare needs of the patient. The plan of care will be continuously reevaluated to ensure continued appropriateness based on the patient’s changing needs and condition.
Responsibilities
Perform nursing process including assessment, care planning, intervention, and evaluation.
Maintain working knowledge of specialized case and utilization management methodologies and practices.
Ensure medical appropriateness criteria (medical necessity) are met and documented from admission through discharge.
Oversee the implementation of an interdisciplinary plan of care with an individualized discharge plan.
Reevaluate the plan of care to ensure continued appropriateness based on the patient’s changing needs and condition.
Essential Skills
Case management
Acute care

Must have 2 years of acute inpatient experience
Trauma 1 level hospital experience
Discharge planning
Additional Skills & Qualifications
RN license
2 years of acute care case management experience
BLS from AHA
Work Environment
The position operates in a trauma level 1 acute care setting with a capacity of 400 beds. The RN Case Manager will work in a collaborative healthcare environment on the day shift.
