Job Title: RN Case ManagerJob Description

The RN Case Manager works efficiently in a fast-paced environment and utilizes clinical expertise, discretion, and independent judgment in performing the nursing process (assessment, care planning, intervention, evaluation) within a patient-centered care professional practice model (Duffy’s Quality-Caring Model) as it relates to case management functions. The RN Case Manager maintains a solid working knowledge of specialized case and utilization management methodologies and practices, applying these concepts to everyday practice. The role ensures that medical appropriateness criteria (medical necessity) are met for the level of care provided and are documented from admission through discharge. The RN Case Manager practices in a collaborative health care environment to oversee the implementation of a well-thought-out interdisciplinary plan of care with an individualized discharge plan that is comprehensive and best meets the continuing healthcare needs of the patient. Additionally, the RN Case Manager reevaluates the plan of care to ensure continued appropriateness based on the patient's changing needs and condition.

Responsibilities

  • Perform the nursing process including assessment, care planning, intervention, and evaluation within a patient-centered care professional practice model.

  • Maintain knowledge of specialized case and utilization management methodologies and practices.

  • Ensure medical appropriateness criteria (medical necessity) are met and documented from admission through discharge.

  • Collaborate with the healthcare team to implement an interdisciplinary plan of care with an individualized discharge plan.

  • Reevaluate the plan of care to ensure continued appropriateness based on the patient's changing needs and condition.

Essential Skills

  • Case management

  • Acute care

  • 2 years of acute inpatient experience

  • Care coordination

  • Trauma 1 level hospital experience

  • Discharge planning

Additional Skills & Qualifications

  • RN license

  • 2 years of acute care case management experience

  • BLS certification from the American Heart Association

Work Environment

This role requires working in a trauma level 1 acute setting with 400 beds. The RN Case Manager will work the day shift in a dynamic and fast-paced environment. The work environment is collaborative and patient-centered, with a focus on delivering high-quality care and ensuring the best outcomes for patients.

Pay and Benefits

The pay range for this position is $50.00 - $70.00
