HAPCAP Community Health Specialist in Logan, Ohio

Hocking Athens Perry Community Action (HAPCAP) is looking for one or more Community Health Specialists to work with our HAPCAP for Health team at the Southeast Ohio Foodbank in Logan, OH. As a member of our team, you will work to further health access, food security, and equity for our rural neighbors.    

The Community Health Specialist will conduct case management in our Bringing Healthy Home (Maternal/Prenatal) and Food as Medicine programs. The Community Health Specialist will meet with program participants on a regular basis, conducting home visits to deliver food and other materials. They will assess goals and assist with accessing resources and supportive services to meet health and social needs. They will also serve as a liaison between participants and healthcare professionals, to help address barriers to healthcare access.

Successful candidates will have the ability to work well with a wide range of staff, partners, neighbors, and vendors. Additionally, the candidate will need to establish and maintain good relationships with community-based organizations, social service agencies, healthcare providers, and managed care providers for referral of participants and care coordination.

A high school diploma or GED is required. Applicants must have a valid driver's license and reliable transportation. In addition, applicants must possess very good computer and organizational skills and have the ability to communicate with people of all socioeconomic backgrounds. A commitment to addressing the causes and conditions of poverty and hunger is essential. Current license as a Community Health Worker in Ohio is preferred but not required.

This is a full-time position within a dynamic, service-oriented work environment. The salary for this position starts and $37,500 annually and includes 100% health, dental, and vision coverage for the employee, a generous paid time off package, retirement plan with 6% employer matching, and periodic Cost of Living Adjustments (COLA).
