POSITION PURPOSE

The Outpatient Infusion Visit Coordinator is responsible for the coordination of all chemotherapy and non-chemotherapy visits within the Cancer Center. This includes the accurate and timely scheduling of various types of therapies according to established protocols while considering the resources available (e.g., clinical team, pharmaceutical supply, infusion chair space, etc). They will work with patients, providers, and the clinical team to ensure complete follow-through from the initial visit-to-final treatment.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

Research individualized therapy plans (chemotherapy, immunotherapy, etc) ensuring patients are scheduled according to established protocol and clinical best practices. Can effectively coordinate the scheduling of a complex patient population within the constraints of clinical team availability, adequate pharmaceutical drug supply and/or blood products, and therapy chair space. Actively engages with the onsite Lead Infusion Nurse Coordinator throughout the day and ongoing, to ensure adequate support of the infusion clinic (e.g., communicating that a new CADD pump treatment is beginning, or ensuring a cancellation is adequately managed) Read and interpret physician office notes to ensure ongoing coordination of extended treatment plans requiring multiple visits over the course of several weeks or months. Coordination of special procedures with our oncology providers including bone marrow biopsies. Ensures coordination of visits adhere to Cancer Research study protocols, where applicable Coordinates patient schedules in collaboration with external departments including radiation oncology, urology, neurology, radiology, etc. Coordinates products from local Blood Bank for blood/platelet transfusions. Can effectively prioritize stat/urgent requests to ensure escalation occurs in timely manner during critical patient situation (e.g., hydration, blood transfusions, and urgent chemotherapies). Understands protocol and can effectively educate a patient on their required lab work prior to each relevant therapy visit. Responsible for creating a hospital account (HAR), assigning referrals from work queue to appointments. Responds to In-basket communications, secure chats, Teams messages, emails, and physician orders (Intent-to-Treat) in EPIC in a timely manner. Uses proper medical terminology to translate clinical information to schedule patients appropriately for treatment. Triages all incoming calls, assesses callers' needs, directs to appropriate department. Practices excellent phone technique. Obtains and communicates messages in an accurate and timely manner. Must be able to consider the patients entire care plan coordinating around other office visits, procedures, and/or tests. Cancels/reschedules appointments according to physician and infusion center guidelines. Communicates to patients/family members regarding infusion appointments. Serves as a liaison between patient and medical staff. Upon completion of chemotherapy, ensure the patient is scheduled for monthly port maintenance. Maintains communication with the Nurse Coordinator at each location daily. Helps refer patients if there is a need for financial counseling, social work, dietician, and Chaplin services as needed. Can schedule and cover for multiple Outpatient Infusion Center locations including Chelsea, Canton, Brighton, Ann Arbor and Livonia. Verifies with the Financial Navigators on insurance approval prior to scheduling. Schedules according to ITT guidelines regarding location, regimen, and when the required treatment is to start by. Performs other duties as assigned by supervisor/designate. Acts independently as well as a team to ensure that all patients get scheduled in a timely manner. Informs patients/visitors of hospital policies. Supports and adheres to Trinity Health values and standards. Maintains good rapport and cooperative relationships. Approaches conflict in a constructive manner. Helps to identify problems, offer solutions, and participate in their resolution. Participates in departmental activities to promote patient satisfaction and performance improvement. Maintains the confidentiality of information acquired pertaining to patient, physicians, colleagues, and visitors to St. Joseph Mercy Health. Discusses patient and hospital information only among appropriate personnel in appropriately private places. Behaves in accordance with the Code of Conduct, Service Excellence Standards, and the Mission, Vision, Values, and policies of SJMH. Assumes responsibility for performance of job duties in the safest possible manner, to assure personal safety and that of coworkers, and to report all preventable hazards and unsafe practices immediately to management.

REQUIRED EDUCATION, EXPERIENCE AND CERTIFICATION/LICENSURE

Education: High school diploma or GED. Associates in Healthcare and/or Certificate in Medical is preferred.

Experience: Minimum of one year of scheduling experience in a health care setting. Preferred experience would include coordination of patient care in a complex care setting, cancer center, specialty practice, or other oncology setting; working with providers and caregivers and use of complex scheduling software.

REQUIRED SKILLS AND ABILITIES

Able to exercise independent judgment to set and prioritize cases and defined workload Basic understanding of medical terminology Basic understanding of reading physician orders Proficient in the use of an EMR software, EPIC preferred Working knowledge with Microsoft Office applications Must demonstrate the ability to learn quickly and handle multiple tasks while remaining calm and focused. Must be detail-oriented and able to follow oral and written instructions. Must be able to maintain accurate records Must be able to communicate effectively with internal and external providers
